Abstract

Latar Belakang: Muscle wasting adalah salah satu manifestasi ekstrapulmoner yang mempengaruhi kualitas hidup pada pasien PPOK, yang disebabkan oleh ketidakseimbangan metabolisme protein pada otot rangka. Study ini bertujuan untuk mengevaluasi efek ekstrak Ophiocephalus striatus pada Neutrofil, suPAR, kapasitas difusi paru (DLCO) dan kualitas hidup pada pasien PPOK yang stabil dengan muscle wasting. Metode: Penelitian klinis dengan metode quasi experimental pada 32 pasien PPOK stabil dengan muscle wasting di Poli Paru Rumah Sakit Umum Dr. Saiful Anwar Malang, berdasarkan kriteria GOLD 2016 dan BIA (Bioelectrical impedance analysis) dengan FFM (fat-free mass) < 14,6 kg/m dan BMI (Body Mass Index) < 18,5 kg/m2, pengukuran kadar suPAR dengan metode ELISA, Neutrofil dengan analisa darah, DLCO dengan Bodypletysmography, dan CAT score sebagai parameter kesehatan PPOK kualitas hidup dilakukan sebelum dan sesudah intervensi gizi Ophiocephalus striatus ekstrak 3000 mg diberikan selama 12 minggu. Hasil: Terjadi penurunan nilai CAT yang signifikan (18,34 ± 8,031 sampai 10,81 ± 7,66, p = 0,000), penurunan suPAR yang tidak signifikan (3,93±2,04 pg / mL menjadi 3,77±1, 67 pg / mL, p = 0,67), penurunan Neutrofil tidak signifikan (61,74±11,54% sampai 60,86±10,46%, p = 0,619),peningkatan yang tidak signifikan kapasitas difusi paru (53,73±24,56% sampai 56,06 ± 25,93%, p = 0,369). Ada korelasi negatif yang lemah antara suPAR terhadap CAT (r = 0, 44; p = 0,813) dan DLCO (r = - 0,170; p = 0,352), korelasi positif lemah antara suPAR terhadap Neutrofil (r = 0,167; p = 0,370). Kesimpulan: Suplementasi 12 minggu ekstrak Ophiocephalus striatus pada pasien PPOK stabil dengan Muscle wasting akan menurunkan kadar suPAR dan Neutrophil secara tidak signifikan dan meningkatkan kapasitas difusi paru (DLCO), namun signifikan pada nilai CAT. Tidak ada korelasi antara perubahan suPAR, Neutrofil, DLCO sampai nilai CAT.

English Abstract

Background : Muscle wasting is one of extrapulmonary manifestation that influence quality of life in COPD patients, caused by imbalanced of protein metabolism in skeletal muscle. This Study aimed to evaluate the effect of Ophiocephalus striatus extract on Neutrophil, suPAR, diffusion capacity of lung for carbon monoxide(DLCO) and quality of life in stable COPD patients with muscle wasting. Methods : Clinical pre and post quasi experimental study to 32 stable muscle-wasted COPD patients from Pulmonary Outpatient Clinic Saiful Anwar Hospital Malang, determined by GOLD 2016, BIA(Bioelectrical impedance analysis) with FFM (Fat Free Mass) < 14,6 kg/m dan BMI (Body Mass Index) < 18,5 kg/m2, measurements of suPAR levels by ELISA, Neutrophil with blood analysis, DLCO with Bodyplethysmography, and CAT score as health parameters COPD quality of life were performed before and after nutritional intervention Ophiocephalus striatus extract 3000 mg were given for 12 weeks. Results : There were significant decreased of CAT score (18,34 ± 8,031 sampai 10,81 ± 7,66, p = 0,000), unsignificant decreased of suPAR (3,932,04 pg/mL to 3,771,67 pg/mL, p=0,67), unsignificant decreased of Neutrophil (61,74±11,54% sampai 60,86±10,46%, p = 0,619), unsignificant Increased of lung diffusion capacity (53.7324,56% to 56.0625,93% p = 0.369). There were weak negative correlations between suPAR to CAT (r = 0, 44; p = 0,813) and DLCO (r=-0,170;p=0,352), weak positive correlation between suPAR to Neutrophil (r=0,167;p=0,370).Conclusio : Twelve weeks supplementation of Ophiocephalus striatus extract in stable COPD patients with muscle wasting unsignificantly decrease suPAR and Neutrophil levels and increase lung diffusion capacity (DLCO), but significant on CAT scores. No correlation between changes in suPAR, Neutrofil, DLCO to CAT scores.
