Abstract
Sepsis neonatorum awitan dini adalah infeksi bakteri berat yang menyebar ke seluruh tubuh bayi baru lahir kurang dari 3 hari. Tujuan penelitian ini adalah untuk mengetahui hubungan antara ditemukan bakteri, peningkatan jumlah neutrofil dan peningkatan kadar IL-6 pada cairan ketuban bayi dengan risiko infeksi dengan terjadinya sepsis neonatorum awitan dini dan untuk melihat bahwa tidak ada perbedaan hasil tes kepekaan terhadap antibiotika antara metode Primary sensitivity tes dan metode baku pada cairan ketuban. Pada penelitian ini digunakan metode studi kasus kontrol dari bahan pemeriksaan cairan ketuban bayi dengan risiko infeksi yang kemudian dilakukan uji kepekaan antibiotika dengan Primary sensitivity tes dan metode baku dan dilakukan kultur, pemeriksan elisa dan Imuositokimia. waktu penelitian 1 juli 2010 - September 2010 didapatkan 81 sampel, dikumpulkan menjadi 25 sampel bayi dengan risiko infeksi, 30 bayi prematur, 26 bayi sehat sebagai kontrol. Didapatkan hasil bahwa ada hubungan antara ditemukan kuman dengan terjadinya sepsis p 0.00,Odds rasio:11,4,CI95% 3,5-35, sensitivitas 81%, spesifisitas 72%,NPN 88%, NPP 81%. Jumlah neutrofil 14/LP berhubungan dengan terjadinya sepsis p 0.001, Odds ratio: ratio:11,7,CI.95% 11.6-17 sensitivitas 77%,spesifisitas 77%,NPN 586%, NPP 56%. Kadar IL-6 30.1 ng/ml berhubungan dengan terjadinya sepsis p 0.001,odd rasio:11,7, CI.95% 11.6-17 sensitivitas 77%, spesifisitas 77%, NPN 586%, NPP 56%. Hasil sensitivitas antibiotika dengan PST dan metode baku adalah sama yaitu , amikasin, meropenem dan cefotaxim. Kesimpulan: ada hubungan antara ditemukan kuman, peningkatan jumlah neutrofil dan IL-6 pada cairan ketuban bayi risiko infeksi dengan terjadinya sepsis neonatorum awitan dini, serta tidak ada perbedaan hasil sensitivitas antibiotika antara PST dan Metode baku.
English Abstract
Neonatal sepsis is a severe bacterial infection that spreads throughout body of neonatus before 3 days. purpose of this research is to know relationship between bacteria findings, increasing neutrophil counts and IL-6 level in amniotic fluid of infants who had risks of infection including early onset neonatal sepsis and to know that re is no significant differences in antibiotic`s sensitivity tests between PST and standard methods in amniotic fluid. This research uses case control study as method from amnion samples of neonates who had risks of infection. refore antibiotic sensitivity tests dan culturing were done. ELISA and immunocytochemistry also done. Eighty-one samples were collected during Juli 1 st , 2010 – september 1 st ,2010 and grouped into twenty-five samples of neonates who had risks of infection, thirty samples of premature infants, twenty-six vigorous infants as control. re is a relationship between sample with bacteria findings and occurance of sepsis, as p 0.00,Odds ratio:11,4,CI95% 3,5-35, sensitivity 81%,specificity72%,NPV 88%, PPV 81%. Neutrophil more than 14 has correlation with occurance of sepsis p 0.001,odds ratio:11,7,CI.95% 11.6-17 sensitivity 88%,specificity 64%,NPV 55%, PPV 64%. Interleukin-6 more than 30.1 ng/mL have relationship with occurance of sepsis p 0.001,odds ratio: ratio:11,7, CI.95%11.6-17 sensitivity 77%, specificity 77%, NPV 586%, PPV 56%. All results from antibiotic sensitivity test by PST and standard method is same.Conclusion: re is a correlation between bacteria findings, increasing neutrophil and IL-6 in neonates amniotic fluid who had risks of infection and occurance of early onset neonatal sepsis. No differences found in antibiotic sensitivity tests between PST and standard method.
