Abstract
Tujuan: Membuktikan adanya perbedaan kadar IL-4 serum pada aktifitas penyakit ringan dan berat, indeks aktifitas rendah dan tinggi, dan indeks kronisitas rendah dan tinggi pada pasien NL. Metode: Merupakan penelitian observasional dengan metode potong lintang. Subyek penelitian adalah semua pasien yang memenuhi kriteria SLE sesuai American College of Rheumatoplogy (ACR) 1997 dan dibuktikan menderita nefritis lupus pada biopsi ginjal. Aktifitas penyakit dinilai dari Renal Domain SLEDAI Score. Indeks aktifitas dan indeks kronisitas dinilai dengan sistem National Institute of Health (NIH). Semua subyek diperiksa kadar IL-4 serum yang kemudian dilakukan uji beda pada masing-masing kelompok dengan menggunakan tes t untuk data dengan distribusi normal, dan menggunakan non parametrik dengan uji Spearman untuk data dengan distribusi tidak normal. Hasil: Terdapat 47 pasien NL dengan kelas I-II sebanyak 10 orang dan kelas III, IV sebanyak 37 orang yang akan dianalisis lebih lanjut. Kemudian dibagi menjadi kelompok aktifitas penyakit ringan (N=24) dan berat (N=13), kelompok indeks aktifitas rendah (N=29) dan tinggi (N=8) serta kelompok indeks kronisitas rendah (N=16) dan tinggi (N=21). Kadar IL-4 berbeda bermakna (p=0.039) dengan kadar IL-4 serum pada aktifitas penyakit ringan adalah 133.17 + 79.93 pg/mL dengan kadar IL-4 serum pada aktifitas penyakit tinggi adalah 86.33 + 70.62 pg/mL. Tidak terdapat perbedaan kadar IL4 serum pada indeks aktivitas ringan dan berat (p=0.599) meskipun kadar IL-4 serum lebih tinggi pada indeks aktifitas rendah (144.49 + 81.79 pg/mL) daripada indeks aktifitas tinggi (124.77 + 73.01). Terdapat perbedaan bermakna (p=0.012) kadar IL-4 serum pada indeks kronisitas rendah dan tinggi dengan kadar IL-4 pada indeks kronisitas rendah lebih rendah (76.16 + 42.32 pg/mL) daripada indeks kronisitas tinggi (147.61 + 87.20 pg/mL). Kesimpulan: Kadar IL-4 serum berbeda bermakna pada aktifitas penyakit ringan dengan aktifitas penyakit berat, serta pada indeks kronisitas rendah dengan indeks kronisitas tinggi, dimana kadar IL-4 serum lebih tinggi pada aktifitas penyakit rendah dan indeks kronisitas tinggi. Tidak didapatkan perbedaan kadar IL-4 serum pada indeks aktifitas rendah dan tinggi.
English Abstract
Objective: To compare levels of IL-4 serum in mild and severe disease activities, low and high activity and chronicity indices in lupus nephritis patients in Saiful Anwar Hospital. Method: This is a cross sectional survey study. Subjects were eligible for inclusion if y fulfilled American College of Rheumatology (ACR) 1997 revised criteria for classification of SLE and had a renal biopsy consistent with lupus nephritis. IL-4 serum level was measured at same day before biopsy was performed using enzyme-linked immunosorbent assay (ELISA). Disease activity was assessed using renal domain SLEDAI scores. Activity index and chronicity index were scored using National Institute of Health (NIH) system. Mild diease activity has renal domain SLEDAI score ≤ 4, low activity index has activity index less n 6, and low chronicity index was defined as chronicity index less n 3. Comparative analysis using t test and non parametric Mann Whitney test were performed. Result: Disease activity, index activity and chronicity was assessed among 37 lupus nephritis patients with class III and IV. re were 24 patients (64.86%) with mild disease activity compared to 13 (35.14%) patients with severe disease activity. 29 patients (78.4%) of low activity index were compared to 8 patients (21.6%) of high activity index, and re were 16 patients (43.2%) with low chronicity index compared to 21 patients (56.8%) with high chronicity index. re was diffrences of IL-4 serum level in mild disease activity compared to severe urinalysis activity with p=0.039, levels of IL-4 serum were higher in mild disease activity (133.17 + 79.93 pg/mL vs 86.33 + 70.62 pg/mL). re were no difference of IL-4 levels in low activity index and high activity index (p=0.599) eventhough IL-4 serum level was higher in low activity index (144.49 + 81.79 pg/mL vs 124.77 + 73.01). level of IL-4 serum was higher in high chronicity index (147.61 + 87.20 pg/mL) n in low chronicity index (76.16 + 42.32 pg/mL) with p=0.012. Conclusion: re were differences of IL-4 serum levels among mild and severe disease activity, and between low and high chronicity index. This study showed IL-4 may have positivity correlation on chronicity index in fibrogenesis process, and reduce disease activity. re was no difference of IL-4 serum levels between low activity index and high activity index.
