Abstract
Background: Tumor necrosis factor (TNF)-α is a proinflammatory cytokine which have been implicated in pathogenesis of rheumatoid arthritis (RA). re were several studies already conducted to established relationship between TNF-α and disease activity parameters and severity of joint destruction, but relationship between TNF-α and disease activity parameters and severity of joint destruction have not been studied yet in Indonesian RA patients. Objective: purpose of this survey cross-sectional study was to investigate correlation between serum TNF-α level with disease activity parameters and severity of joint destruction in RA patients at Rheumatology Outpatient Clinic, Saiful Anwar General Hospital, Malang, Indonesia. Methods: We studied 24 patients with RA, fulfilling revised criteria of ACR 1987. Clinical parameters and disease activity measurements were collected such as age, sex, disease duration and diseases activity score (DAS28). Laboratory parameters were hemoglobin, rheumatoid factor (RF) and C-reactive protein (CRP). Serum TNF-α was measured with enzyme linked immunosorbent assay ( ELISA). Severity of joint destruction was assessed using bilateral postero-anterior hand x ray (Sharp score). Results: average of serum TNF-α level was 95,38 + 83,03 pg/mL Serum TNF-α was correlated significantly with disease activity parameters, such as DAS28 (p=0.008, r=0.530), tender joint count (TJC) (p=0.014, r=0.497), swollen joint count (SJC) (p=0.031, r=0.441), and CRP (p=0.007, r=0.539). re were no correlation between serum TNF-α level with Sharp score. Conclusion: Serum TNF-α level was significantly correlated with disease activity parameters, but not with joint destruction, in RA patients. Our data may support recent evidence that TNF-α have a significant role in activity of RA.
English Abstract
Latar Belakang: Tumor necrosis factor (TNF)-α merupakan sitokin proinflamasi yang banyak terlibat dalam patogenesis artriris reumatoid (AR). Beberapa penelitian sebelumnya telah menunjukkan hubungan antara TNF-α dan parameter aktivitas penyakit dan derajat kerusakan sendi, akan tetapi hubungan antara TNF-α dengan aktivitas penyakit dan kerusakan sendi sepanjang pengetahuan kami belum pernah diteliti pada pasien AR di Indonesia. Tujuan: Tujuan dari penelitian survei potong-lintang ini ialah untuk mengetahui korelasi antara kadar TNF-α serum dengan parameter aktivitas penyakit dan derajat kerusakan sendi pada pasien AR di Poli Rematologi , RSU dr. Saiful Anwar, Malang, Indonesia. Metode: Penelitian ini melibatkan 24 pasien AR, yang memenuhi kriteria revisi ACR 1987. Parameter klinik dan pengukuran aktivitas penyakit dikumpulkan. Seperti umur, jenis kelamin, lama sakit dan disease activity score (DAS28). Parameter laboratorium yang diukur antara lain hemoglobin, rheumatoid factor (RF) dan C-reactive protein (CRP). Serum TNF-α diukur dengan metode enzyme linked immunosorbent assay ( ELISA). Derajat kerusakan sendi diukur dengan menggunakan bilateral postero-anterior x ray manus (skor Sharp). Hasil: Rata-rata kadar TNF-α serum ialah 95,38 + 83,03 pg/mL. Kadar TNF-α serum berkorelasi secara bermakna dengan parameter aktivitas penyakit, antaralain DAS28 (p=0.008, r=0.530), tender joint count (TJC) (p=0.014, r=0.497), swollen joint count (SJC) (p=0.031, r=0.441), dan CRP (p=0.007, r=0.539). Tidak didapatkan korelasi yang bermakna antara kadar TNF-α serum dengan skor Sharp. Kesimpulan: Kadar TNF-α serum berkorelasi secara signifikan dengan parameter aktivitas penyakit, tetapi tidak didapatkan korelasi yang bermakna dengan derajat kerusakan sendi pada pasien AR. Data penelitian kami menunjang bukti hasil penelitian-penelitian sebelumnya yang menunjukkan peran penting TNF-α pada aktivitas AR.
