English Abstract

Background : Diabetic nephropathy (DN) is leading cause of end-stage renal disease. It progress from early stages marked by microalbumiurine to later stages characterized by macroalbuminurine. One of key components of glomerular filtration slit spanning between neighboring podocytes is nephrin. Its expression is altered in experimental models of diabetes and also in various human proteinuric diseases, including diabetes. Objectives : We investigated association between nephrinurine and level of albuminurine in type-2 diabetic patients. Patient and Methods : A cross-sectional study which datas were collected from type-2 diabetic patients with normal RFT from endocrinology outpatient department of Dr. Syaiful Anwar General Hospital Malang. Albuminuria was measured in random spot urine samples using albumin/creatinine ratio (ACR), while nephrinuri was screened with western blotting using anti-nephrin antibody. Analysis of Diabetic control include FBG, 2 hours-PPBG, lipid profiles and HbA1c level. Results : Type-2 diabetic patients with normoalbuminuria (n=26), with microalbuminuria (n= 22), and with macroalbuminuria (n= 3) were screened for nephrinuria . Nephrinuria was present in 61,5% of normoalbuminuria, 77,3% of microalbuminuria, and 100% of macroalbuminuria. Statistical analysis revealed no relationship between nephrinuria and level of albuminuri. In addition, from clinical parameters, level of albuminuri excretion rate was significantly correlated with duration of diabetes (p=0.009, r=0.063), FBG (p=0.037, r=0.293), plasma creatinine (p=0.003, r=0.402), and MDRD eGFR (p=0.003, r= -0.402), and cholesterol HDL (p=0.043, r= -0.284). Conclusion : Nephrin bands were obtained in 70.6% of research subjects, of which half are still having a normal ACR. percentage of subjects with nephrinuri was increasing with increasing level of albuminuri excretion rate. Statistical analysis revealed no correlation between level of albuminuri and nephrinuri. It is still a discussion whe r nephrinuri can be an early predictor of renal dysfunction in DN before onset of microalbuminuri.
