Abstract

Rinitis alergi merupakan masalah kesehatan global dan dapat mengganggu kualitas hidup penderitanya. Beberapa penelitian telah menunjukkan peran leptin, hormon yang diproduksi oleh jaringan lemak, pada proses sensitisasi yang ditandai dengan kemampuan leptin meningkatkan sel B dan IgE namun peran leptin pada fase klinis masih belum diketahui. Tujuan: Penelitian ini bertujuan untuk mengetahui perbedaan kadar leptin serum penderita rinitis alergi dan tanpa alergi serta hubungan kadar leptin serum dengan eosinofil mukosa hidung dan derajat rinitis alergi. Metode dan bahan: Penelitian dilakukan dengan desain case control untuk mengetahui perbedaan kadar leptin serum pada penderita rinitis alergi dan tanpa alergi yang masing-masing 38 subyek. Untuk mengetahui hubungan kadar leptin serum dengan eosinofil mukosa hidung dan derajat rinitis alergi pada penderita rinitis alergi dilakukan desain cross sectional . Kadar leptin serum diperiksa menggunakan ELISA. Eosinofil mukosa hidung diambil dari mukosa hidung dan diberi warna dengan May-Grunwald-Giemsa kemudian dihitung per 100 sel. Derajat rinitis alergi ditentukan berdasarkan ARIA dan VAS. Analisa statistik yang digunakan pada penelitian ini adalah uji t, uji regresi, dan Kruskal-Wallis. Hasil penelitian: Hasil menunjukkan kadar leptin serum penderita rinitis alergi (24,9 ng/ml, SD=3,92) lebih tinggi secara bermakna (p 0,05) dibandingkan dengan kontrol (20,4 ng/ml, SD=2,44). Terdapat hubungan positif yang bermakna (r=0.453; p 0,05) antara kadar leptin serum dengan jumlah eosinofil mukosa hidung. Kadar leptin serum berhubungan dengan derajat rinitis alergi berdasarkan ARIA (p 0,05) khususnya dengan intensitas keluhannya (p 0,05) namun tidak berhubungan dengan derajat rinitis berdasarkan VAS. Kesimpulan dan saran: Leptin terbukti mempunyai peran secara spesifik pada terjadinya rinitis alergi. Secara spesifik kadar leptin serum berhubungan dengan jumlah eosinofil mukosa hidung dan derajat rinitis alergi, terutama pada intensitas keluhan dan bukan pada beratnya keluhan. Pengendalian kadar leptin serum dapat dipertimbangkan sebagai upaya mencegah dan memperbaiki kesehatan penderita rinitis alergi. Penelitian lebih lanjut yang menekankan pada pengendalian kadar serum leptin disarankan untuk mengendalikan keluhan rinitis alergi.

English Abstract

Allergic rhinitis is a global health problem that can impair quality of life. Recent studies have identified role of leptin, hormone that produced by adipocyte tissue, on sensitization process by increasing B cells and IgE but its role in clinical aspect was not well known yet. Objectives: This study was aimed to investigate role of leptin in allergic rhinitis by determining difference of serum leptin levels between patients with and without allergic rhinitis, correlation of serum leptin levels with eosinophil of nasal mucosa membrane and degree of allergic rhinitis. Methods and material: A case control was used in this study to identify differences of serum leptin levels in 38 patients with allergic rhinitis and without allergy. A cross sectional design to determine correlation of serum leptin levels with eosinophil of nasal mucosa membrane and degree of allergic rhinitis was conducted. Serum leptin levels were measured using ELISA. Nasal mucosa membrane eosinophil was taken by scraping nasal mucosa membrane and colouring it with May-Grunwald-Giemsa. Eosinophil was counted in each 100 cells. ARIA and VAS were applied to determine degree of allergic rhinitis. Statistical analyses in this research were included t-test, linier regression, and Kruskal-Wallis. Results: Serum leptin levels in allergic rhinitis patients (24,9 ng/ml, SD=3,92) was significantly higher (p 0,05) than control (20,4 ng/ml, SD=2,44). re was significantly positive correlation (r=0.453; p 0,05) between serum leptin levels and eosinophil of nasal mucosa membrane. Serum leptin levels also had a correlation with degree of allergic rhinitis based on ARIA (p 0,05), specifically on intensity of symptom (p 0,05), but re was no correlation with severity of allergic rhinitis based on VAS. Conclusions and recommendation: Leptin had a specific role on patophysiology of allergic rhinitis. Serum leptin levels had a correlation with eosinophil of nasal mucosa membrane and degree of allergic rhinitis, specifically on intensity of symptom but not on severity of symptom. refore, controlling serum leptin levels should be considered as a potential strategy to improve health status of people with allergic rhinitis. Fur r research focusing on serum leptin levels control that can contribute to severity of allergic rhinitis is recommended.
