Abstract

Preeklampsia merupakan salah satu penyebab kematian ibu. Pada preeklampsia terjadi disfungsi endotel memicu munculnya sitokin inflamasi dan meningkatkan ekspresi molekul adhesi ICAM-1 dan VCAM-1. Inflamasi pada preeklampsia bisa dikendalikan dengan pemberian antosianin. Buah Juwet (Syzygium cumini) mengandung antosianin yang berfungsi sebagai antioksidan dan antiinflamasi. Penelitian ini bertujuan membuktikan pengaruh pemberian ekstrak Juwet (Syzygium cumini) terhadap ekspresi molekul adhesi 1 (ICAM-1) dan molekul adhesi sel vaskuler 1 (VCAM-1) pada model HUVECs yang dipapar plasma preeclampsia. Desain penelitian adalah penelitian eksperimental (experimental) yang dikerjakan di laboratorium secara in vitro. HUVECs dipapar 2% plasma preeclampsia sebagai model HUVECs preeclampsia secara in vitro. Kelompok 1 (control negative): HUVECs; 2. (control positif): Model HUVECs preeclampsia+inkubasi 1 jam; 3.Model HUVECs preeclampsia+inkubasi 3 jam;4.Model HUVECs preeclampsia+inkubasi 24 jam; 5 Model+ekstrak Juwet 100ppm+inkubasi 1 jam; 6. Model+ekstrak Juwet 200ppm+inkubasi 1 jam; 7. Model+ekstrak Juwet 400ppm+inkubasi 1 jam; 8. Model+ekstrak Juwet 100ppm+inkubasi 3 jam; 9. Model+ekstrak Juwet 200ppm+inkubasi 3 jam; 10. Model+ekstrak Juwet 400ppm+inkubasi 3 jam; 11. Model+ekstrak Juwet 100ppm+inkubasi 24 jam; 12. Model+ekstrak Juwet 200ppm+inkubasi 24 jam; 13. Model+ekstrak Juwet 400ppm+inkubasi 24 jam. Pemeriksaan ekpresi ICAM-1 dan VCAM-1 menggunakan immunohistokimia. Analisa statistic menggunakan uji Two Way Anova kemudian untuk mengetahui pemaparan mana yang menunjukan perbedaan dilakukan uji LSD dengan tingkat kepercayaan 95%. Ekspresi ICAM-1dan VCAM-1 dengan waktu inkubasi 1 jam, 3 jam, dan 24 jam, diperoleh ada perbedaan yang sangat bermakna rerata VCAM-1 maupun ICAM-1 keduanya menunjukkan p 0.05. Pada waktu Inkubasi tidak ada perbedaan yang bermakna rerata VCAM-1 (p=0.98 ) maupun ICAM-1 (p=0.3920 ). Pada dosis Juwet ada perbedaan yang bermakna rerata VCAM-1 (p=0.000 ) maupun ICAM-1 (p=0.000 ). Sedangkan waktu inkubasi dan dosis Juwet ada perbedaan yang sangat bermakna rerata VCAM-1 (p=0.534 ) maupun ICAM-1 (p=0.285 ). Kesimpulan: ekspresi ICAM-1 dan VCAM-1 meningkat pada kondisi preeklampsi, pemberian juwet dengan dosis 100 ppm, 200 ppm dan 400 ppm dapat menurunkan ekspresi ICAM-1 dan VCAM-1 pada kondisi preeclampsia.

English Abstract

Preeclampsia is one of causes of maternal mortality. Endo lial dysfunction in preeclampsia trigger increases expression of inflammatory cytokines and adhesion molecules ICAM-1 and VCAM-1. Inflammation in preeclampsia can be controlled by administration of anthocyanin. Juwet fruit (Syzygium cumini) contain anthocyanins which act as antioxidants and anti-inflammatory. This study aims to prove effect of juwet extract (Syzygium cumini) on expression of adhesion molecule 1 (ICAM-1) and vascular cell adhesion molecule 1 (VCAM-1) on model of preeclampsia HUVECs were exposed to plasma. study design was experimental research (experimental) were done in laboratory in vitro. HUVECs exposed to 2% plasma preeclampsia as a model of preeclampsia HUVECs in vitro. Group 1 (negative control): HUVECs; 2. (Positive control): Model HUVECs preeclampsia + 1 hour incubation; 3.Model preeclampsia HUVECs + 3 hours of incubation; 4.Model preeclampsia HUVECs + 24 hours of incubation; 5 models juwet 100ppm + extract + 1 hour incubation; 6. Model juwet 200ppm + extract 1 hour incubation; 7. Model juwet 400ppm + extract 1 hour incubation; 8. Model 100ppm + extracts juwet 3 hour incubation; 9. Model 200ppm + extracts juwet 3 hour incubation; 10. Model 400ppm + extracts juwet 3 hour incubation; 11. Model juwet extract 100ppm + 24-hour incubation; 12. Model juwet extract 200ppm + 24-hour incubation; 13. Model juwet extract 400ppm + 24-hour incubation. Examination of expression of ICAM-1 and VCAM-1 using immunohistochemistry. Statistical analysis using Two Way Anova test later to find out where exposure is done Tukey test showed differences with 95% confidence level. expression of ICAM-1 and VCAM-1 with incubation time 1 hour, 3 hours, and 24 hours, re was obtained a very significant difference in mean VCAM-1 and ICAM-1 both indicate P 0.05. At time of incubation no significant difference in mean VCAM-1 (p = 0.98 α) and ICAM-1 (p = 0.3920 α). At doses juwet no significant difference mean VCAM-1 (p = 0.000 α) and ICAM-1 (p = 0.000 α). While incubation time and dose juwet re were a very significant difference in mean VCAM-1 (p = 0.534 α) and ICAM-1 (p = 0.285 α). Conclusion: expression of ICAM-1 and VCAM-1 were increased in conditions preeclampsi, juwet administration at a dose of 100 ppm, 200 ppm and 400 ppm can decrease expression of ICAM-1 and VCAM-1 in preeclampsia condition.
