Abstract

Inflammatory Bowel Disease (IBD) adalah penyakit inflamasi kronik pada saluran pencernaan terutama usus halus dan usus besar. Salah satu faktor penyebab IBD adalah gangguan respon imun yang dapat dirangsang oleh adanya agen bakteri, virus dan xenobiotik. Salah satu penyebab gangguan respon imun dalam usus halus adalah indometasin yang termasuk dalam NSAID ( non-steroidal anti-inflammatory drug ). Indometasin dapat menstimulasi produksi ROS sehingga diperlukan antioksidan pada terapi IBD. Penelitian ini bertujuan untuk menjelaskan potensi ekstrak air daun kedondong dalam menurunkan kadar malondialdehid (MDA) dan aktivitas protease, memperbaiki ekspresi protein ZO-1 dan occludin, menurunkan ekspresi PKC-α dan iNOS serta memperbaiki jaringan ileum tikus yang dipapar indometasin. Penelitian ini menggunakan hewan coba tikus putih ( Rattus norvegicus ) jantan galur wistar dengan berat sekitar 125-175 gram. Kelompok tikus sakit (IBD) diinduksi indometasin dengan dosis 15 mg/kg BB satu kali secara oral dan diinkubasi selama 14 hari. Tikus diterapi dengan ekstrak air daun kedondong dengan dosis 100 mg/ kg BB tikus selama 14 hari berturut-turut. Senyawa-senyawa yang terkandung pada ekstrak air daun kedondong diidentifikasi menggunakan uji fitokimia, Kromatografi Lapis Tipis dan Spektrofotometri Infra Merah. Hasil penelitian menunjukkan bahwa hasil uji fitokimia senyawa ekstrak air daun kedondong ( Lannea coromandelica ) mengandung senyawa flavonoid, alkaloid dan tanin. Hasil penelitian menunjukkan adanya penurunan kadar MDA dan aktivitas protease berturut-turut sebesar 156,16% dan 163,98%. Selain itu, terdapat perbaikan ekspresi protein ZO-1 dan occludin sebesar 46,64% dan 75,28% serta penurunan ekspresi PKC-α dan iNOS 55,57% dan 72,22%. Berdasarkan pengamatan histologis preparat ileum, menunjukkan adanya perbaikan jaringan ileum tikus model IBD. Hasil analisis statistik kadar MDA, aktivitas protease, ekspresi ZO-1, Occludin, iNOS dan PKC-α menunjukkan bahwa terdapat perbedaan sangat nyata antara tikus kontrol negatif, tikus sakit yang diinduksi indometasin (IBD) dan tikus yang diterapi ekstrak air daun kedondong ( Lannea coromandelica ) (p 0,05).

English Abstract

Infammatory Bowel Disease (IBD) is a chronic inflammatory disease of digestive tract especially in small and large intestines. IBD is caused by immune response disturbance which stimulated by virus, bacteria and xenobiotic. One of which stimulate immune response disturbance in small intestine is indomethacin as NSAID ( non-steroidal anti-inflammatory drug ). Indomethacin can stimulate ROS production so that need antioxidant for IBD rapy. aims of this research were to describe potency of Lannea coromandelica leaves aquoeous extract to decrease malondialdehyde (MDA) level and protease activity, to repair both of ZO-1 and occludin protein expressions, to decrease PKC-α and iNOS expression and rat small intestine ileum tissue damage which inducted by indomethacin. This study used experimental animals of male white rat ( Rattus norvegicus ) wistar strain which have 125-175 gram of weight. Group of sick rat (IBD) induced by indomethacin at a dose 15 mg/kg body weight once orally and incubated for 14 days. rat was treated with Lannea coromandelica leaves aqueous extract with a dose 100 mg/kg weight of rat for 14 days consecutively. Based on phytochemistry test, TLC and IR analysis. result showed that Lannea coromandelica leaves aqueous extract contains flavonoids, tanin and alkaloid. MDA level and protease activity were respectively decreased, 156,16% and 163,93%. In addition, re were retrieval of ZO-1 and occludin protein expressions 46,64% and 75,28% , decrease PKC-α and iNOS expression up to 55,57% dan 72,22% as well as retrieval of small intestine ileum tissue damage of rat by histological observation. Statistical analysis indicated that MDA level and protease activity , expression of ZO-1, Occludin, iNOS and PKC- α were extremely difference (p 0.05) on negative control rat, indomethacininduced (IBD) rat and rapy rat with Lannea coromandelica leaves aqueous extract.
