Abstract

Sejauh ini klinisi menggunakan foto toraks untuk menggambarkan beratnya tuberkulosis paru selain dengan pemeriksaan sputum BTA. Apakah keadaan tersebut sesuai dengan stres oksidatif yang terjadi dengan parameter oksidan Malondialdehyde (MDA) dan antioksidan Superoxide Dismutase (SOD)? Sehingga dilakukan penelitian yang bertujuan untuk mengetahui kadar Malondialdehyd dan aktivitas Superoksid Dismutase yang berperan dalam proses kerusakan jaringan paru pada penderita tuberkulosis paru dengan gambaran radiologis lesi minimal dan penderita tuberkulosis paru lesi luas. Metode . Desain penelitian observasional cross sectional terhadap 2 kelompok yaitu penderita TB paru lesi minimal dan TB paru lesi luas, masing-masing terdiri dari 10 subjek penelitian. Hasil. Rerata kadar MDA plasma TB paru lesi luas (12,516±3,739 μg/μl) lebih tinggi dibandingkan TB paru lesi minimal (10,582±2,277 μg/μl) dengan perbedaan yang tidak signifikan bermakna (p=0,145). Rerata kadar MDA sputum TB paru lesi luas ( 2,304 ±1,029 μg/μl) hampir sama dengan TB paru lesi minimal (2,522±1,454 μg/μl) dengan perbedaan yang tidak signifikan bermakna (p=0,703). Rerata aktivitas SOD plasma TB paru lesi minimal (41,201±5,217 unit/cc) lebih tinggi pada daripada TB paru lesi luas (25,264 ±11,346 unit/cc) dengan perbedaan yang signifikan bermakna (p=0.00). Signifikan bila p 0,05. Kesimpulan . Pada kelompok TB paru dengan gambaran foto toraks lesi luas terdapat peningkatan rerata kadar MDA dan penurunan aktivitas SOD dibandingkan kelompok TB paru lesi minimal. Luasnya lesi pada foto toraks juga menggambarkan keadaan stres oksidatif yang dapat ditunjukkan dengan peningkatan kadar MDA dan penurunan aktivitas SOD.

English Abstract

At this time, Clinicians use Chest X-Ray to describe severity of pulmonary TB beside of AFB sputum examination. Does this condition relate with oxidative stress with parameter of oxidant Malondialdehyde (MDA) and antioxidant Superoxide Dismutase (SOD)? This study aims to determine level of Malondialdehyd and activity of Superoksid Dismutase which its role in lung parenchym destruction process in pulmonary TB patients group with radiologic appearence minimal lesion and pulmonary TB patients with advanced lesion. Methods . Study design is cross sectional observational for 2 groups : minimal lesion pulmonary TB and advanced lesion pulmonary TB, each consisting of 10 subjects research. Result. Plasma MDA levels in advanced lesion pulmonary TB (12,516±3,739 μg/μl) were higher than minimal lesion pulmonary TB (10,582±2,277 μg/μl) and the levels of plasma MDA did not change significantly (p=0,145). Sputum MDA levels in in advanced lesion pulmonary TB (2,304 ±1,029 μg/μl) were similarly with minimal lesion pulmonary TB( 2,522±1,454 μg/μl) and the level of sputum MDA di not change significantly (p=0,703). Plasma SOD activity in minimal lesion pulmonary TB (41,201±5,217 unit/cc) were higher than advanced lesion pulmonary TB (25,264 ±11,346 unit/cc) and it were significantly increased (p=0,00). Significant if p 0,05. Conclusion . Increase of plasma MDA levels and decrease of SOD activity in advanced lesion pulmonary TB were higher than minimal lesion pulmonary TB. Extension of lesios in Chest X-Ray determine oxidative stress that can be assesed with increase of MDA level and decrease of SOD activity.
