Abstract
Diabetes melitus (DM) disebabkan kekurangan insulin bersifat absolut atau relatif yang ditandai dengan kondisi hiperglikemia. Pada penderita DM juga mengalami abnormalitas metabolisme lemak. Aktivitas pemecahan lemak berada dalam kondisi tidak terkendali sehingga menyebabkan tingginya kadar asam lemak bebas, trigliserida dan kolesterol yang memicu resiko komplikasi penyakit kardiovaskuler.Terapi DM diberikan kepada penderita dengan target minimal dapat menurunkan kadar glukosa darah menjadi normal dan diharapkan dapat mengurangi resiko komplikasi kardiovaskuler. Kulit salak selama ini hanya dianggap sebagai limbah. Hasil uji fitokimia yang dilakukan mengenai kulit buah salak terdapat kandungan antioksidan seperti flavonoid, tannin dan sedikit alkaloid. Berdasarkan latar belakang di atas, akan dikembangkan minuman herbal yang memiliki efek positif terhadap kesehatan. Formulasi terbaik dari teh herbal kombinasi kulit salak, pandan wangi dan kayu manis selanjutnya diuji secara in vivo untuk mengetahui pengaruhnya terhadap kadar glukosa darah dan profil lipid tikus wistar jantan diabetes melitus yang diinduksi aloksan. Penelitian ini dilakukan melalui 2 tahap. Tahap I menentukan formulasi terbaik teh herbal kulit salak menggunakan Rancangan Acak Kelompok (RAK) terdiri dari 2 faktor, 3 level dan tiap perlakuan diulang sebanyak 3 kali sehingga didapat 27 satuan perlakuan. Faktor I=rasio filtrate kulit salak:pandan wangi dan faktor II = rasio penambahan filtrat kayu manis. Tahapan II: pengujian secara in vivo pengaruh teh herbal kulit salak terhadap kadar glukosa darah dan profil lipid dengan desain penelitian Pre and Post Test Only with Control Group dan pemilihan objek penelitian untuk pengelompokan dan pemberian perlakuan menggunakan rancangan tersarang (Nested design) dengan 2 faktor. Pemilihan perlakuan terbaik menggunakan metode Zeleny. Analisa keragaman diuji dengan ANOVA, beda nyata antar perlakuan dilanjutkan dengan uji DMRT atau BNT 5%. Hasil penelitian yang telah dilakukan, pembuatan minuman teh herbal kulit salak formulasi filtrat kulit salak:filtrat daun pandan wangi (90%:10%) dan penambahan konsentrasi filtrat kayu manis 4% menunjukkan kadar antioksidan tertinggi sebesar 76,62%, nilai IC50 sebesar 561,111 ppm dan total fenol sebesar 166,020 μgGAE/ml demikian juga dengan hasil organoleptik menujukkan nilai yang paling baik dari segi rasa, aroma dan warna serta dari penentuan perlakuan terbaik Metode Zeleny juga memberikan nilai yang paling rendah sebesar 0,058. Selain itu, penelitian secara in vivo menunjukkan bahwa perlakuan pemberian teh kulit salak memberikan pengaruh nyata (α=0,05) terhadap penurunan kadar glukosa darah dan profil lipid. Hasil uji penurunan kadar glukosa darah dan profil lipid menunjukkan bahwa selisih penurunan kadar glukosa darah dan profil lipid paling besar pada diberikan oleh pengaruh perlakuan pemberian teh herbal kulit salak dosis 2 (7,2ml/200g BB). Penurunan kadar glukosa darah mencapai 54,43%, total koleterol sebesar 51,09%, kadar trigliserida sebesar 49,44%, kenaikan kadar HDL sebesar 41,65% dan penurunan kadar LDL mencapai 81,57%.
English Abstract
Diabetes mellitus (DM) is caused by absolute or relative insulin deficiency that is characterized by hyperglycemia conditions. DM patients have abnormal fat metabolism too. Activity breakdown of fat are in uncontrolled conditions that lead to high levels of free fatty acids, triglycerides and cholesterol. It leads the risk of diabetes complication is cardiovaskuler disease. Therapy of DM patients have target that it can be lower blood glucose levels to normal value and decrease the risk of cardiovascular complications . Snake fruit’s bark is only considered as waste. Phytochemical test results are performed that snake fruit’s bark contains antioxidants such as flavonoids, tannins and alkaloids. Based on this background, the researchers will develop herbal drink that has a positive effect on health. The best formulation of herbal tea is combination of snake fruit’s bark, cinnamon and fragrant pandan, subsequently tested in vivo to determine the effect on blood glucose levels and lipid profiles of male wistar rats with diabetes mellitus induced alloxan . This research is consist by two stages. The first phase determine the best formulation of herbal tea of snake fruit’s bark using a randomized block design consists of 2 factors , 3 levels and each treatment was repeated 3 times to get 27 units of treatment. First factor = ratio filtrate of snake fruit’s bark: fragrant pandan and second factor = ratio of the addition of cinnamon filtrate. the second stage is testing the in vivo effect of herbal tea of snake fruit’s bark on blood glucose levels and lipid profile using Pre and Post Test Only with Control Group design and object selection on this research for grouping and intervention using nested design with two factors. The best treatment selection uses Zeleny method. Analyze of variants are tested with one-way ANOVA, significant difference between treatments are followed by DMRT or LSD 5 %. The results of this research are the manufacture of herbal tea of snake fruit’s bark has best formulations with ratio of filtrate : fragrant pandan filtrate ( 90 % : 10 % ) and the addition of cinnamon filtrate concentration of 4% show the highest antioxidant content of 76.62 % , the value of IC50 of 561.111 ppm and total phenols of 166.020μgGAE/ml. The organoleptic results show the best value in terms of taste, aroma and color as well as determining the best treatment of Zeleny method gives the lowest value of 0.058. In vivo studies show that herbal tea of snake fruit’s bark treatment gives significant effect ( α = 0.05 ) to decrease blood glucose levels and lipid profiles . The results decrease levels of blood glucose and lipid profile show that the difference reduction in blood glucose levels and lipid profile were the greatest treatment effect is given by second dose of herbal tea based on snake fruit’s bark (7.2 ml/200g BB). The decreasing of blood glucose levels reach 54.43%, 51.09% of the cholesterol total, triglyceride levels by 49.44%, the increasing of HDL levels by 41.65 % and decreasing of LDL levels up to 81.57 %.
