Abstract

Acute Kidney Injury (AKI) merupakan gangguan fungsi ginjal yang terjadi secara cepat yang ditandai peningkatan kadar blood urea nitrogen (BUN) dan kreatinin dalam darah. AKI dapat disebabkan oleh zat kimia nefrotoksik seperti karbon tetraklorida (CCl4). Karbon tetraklorida menyebabkan kerusakan pada glomerulus dan tubulus melalui pembentukan radikal bebas sebagai hasil metabolisme. Ekstrak kapulaga hijau (Elettaria cardamomum) mengandung senyawa flavonoid dengan potensi sebagai antioksidan dan menghambat peroksidasi lipid melalui aktivitas superoxide radical scavenging. Penelitian ini bertujuan mengetahui pengaruh terapi ekstrak kapulaga hijau terhadap kadar BUN dan kreatinin serta perubahan histopatologi ginjal pada tikus model AKI. Tikus model AKI dihasilkan melalui induksi 1,9 g/kg BB CCl4 50% secara intraperitoneal. Penelitian ini menggunakan tikus (Rattus novergicus) jantan berumur 10-12 minggu terbagi atas lima kelompok perlakuan, yaitu kontrol negatif, kontrol positif (AKI), terapi dosis 200 mg/kg BB, 400 mg/kg BB, dan 600 mg/kg BB. Kadar BUN dan kreatinin diukur dengan metode spektrofotometri dan dianalisis secara statistik, sedangkan gambaran histopatologi ginjal dianalisis secara kualitatif. Hasil penelitian menunjukkan bahwa ekstrak kapulaga hijau mampu menurunkan kadar BUN dan kreatinin tikus secara signifikan (p<0,05) serta memperbaiki gambaran histopatologi ginjal dengan dosis terbaik sebesar 600 mg/kg BB, yang dapat menurunkan kadar BUN dan kreatinin sebesar 21,3% dan 41,5%. Perbaikan histopatologi ginjal ditandai dengan tidak adanya edema interstitial, degenerasi hidropik, dan nekrosis epitel tubulus. Kesimpulan penelitian ini adalah ekstrak kapulaga hijau dapat dimanfaatkan sebagai terapi alternatif herbal pada tikus model AKI.

English Abstract

Acute Kidney Injury (AKI) is a rapidly kidney disorder which followed by increasing of blood urea nitrogen (BUN) and creatinine level in blood. AKI can be caused by a nephrotoxic substance like carbon tetrachloride (CCl4). Carbon tetrachloride which affect glomerular and tubular damages, was resulting by free radical formation as metabolic products. The cardamom extract (Elettaria cardamomum) contains flavonoid which can act as antioxidant and reduce lipid peroxidation by its superoxide radical scavenging activitiy. This reaserch was aimed to determine the therapeutic effect of cardamom extract toward BUN and creatinine level and kidney histopathology in AKI rat model. AKI rats model was induced by 1,9 g/kg BW CCl4 50% intraperitoneally. This research used 10-12 weeks male rats (Rattus novergicus) which divided into five treatment groups. These five groups are negative control group, positive control group (AKI), therapeutic group with dose 200 mg/kg BW, 400 mg/kg BW, and 600 mg/kg BW. BUN and creatinine level were measured by spectrophotometry method and analyzed according to statistic, and kidney histopathology was analyzed descriptively. The result show that green cardamom extract therapy decrease BUN and creatinine level significantly (p<0,05) and improve kidney histopathology with optimal dose at 600 mg/kg BB, which decrease BUN and creatinine level at 21,3% and 41,5% respectively. Kidney histopathology improvement was shown by the absence of interstitial edema, hydropic degeneration, and tubular epithelial necrosis. In conclusion green cardamom extract can be utilized as alternative herbal therapy at AKI rats model.
