Abstract
Penelitian ini dilakukan di PT. PAL INDONESIA, tepatnya di departemen QA dan Standarisasi. Obyek dari penelitian ini adalah KM. Pagerungan dengan kapasitas 17.500 Dead weight Tonnage (DWT). Data yang digunakan adalah hasil inspeksi class/Owner Surveyor (OS) terhadap proses assembly KM. Pagerungan. Berdasarkan hasil rekapitulasi menunjukkan bahwa jumlah cacat terbesar ada pada bagian Hull Construction (HC) dengan jumlah cacat sebanyak 129, kemudian diikuti dengan Hull Outfitting (HO) 60 cacat, lalu Machinery Outfitting (MO) sebanyak 32 cacat dan Electrical Outfitting (EO) sebanyak 22 cacat. Dengan berlandaskan data tersebut, maka perlu dilakukan analisis secara mendalam mengenai jenis-jenis cacat yang paling berpengaruh terhadap tingginya jumlah cacat di bagian HC sehingga dapat menghasilkan solusi perbaikan yang bersifat implementatif. Untuk menentukan faktor-faktor yang paling berpengaruh terhadap tingginya jumlah cacat di bagian HC, maka digunakanlah metode FMEA yang menghasilkan output berupa nilai RPN untuk seluruh jenis cacat yang terjadi pada bagian HC. Cacat dengan nilai RPN paling berpengaruh akan menjadi fokus utama untuk diolah dengan menggunakan metode FTA, sehingga dapat diketahui penyebab dari jenis cacat tersebut. Berdasarkan hasil pengolahan FTA, maka dirancanglah saran perbaikan yang sesuai dengan penyebab dari jenis cacat yang ada. Pengolahan data dengan metode FMEA menunjukkan bahwa terdapat tiga jenis cacat dengan nilai RPN tertinggi, yaitu missing bracket dengan nilai RPN 384, missed weld dengan nilai RPN 240 dan misalignment dengan nilai RPN 224. Ketiga jenis cacat ini yang menjadi fokus utama untuk diolah dengan menggunakan metode FTA. Dari pengolahan metode FTA menunjukkan bahwa ketiga jenis cacat ini memiliki keterkaitan dalam hal penyebab utama, yaitu disebabkan karena kesalahan manusia dan proses kontrol yang belum optimal. Berdasarkan hal tersebut maka dirancanglah beberapa saran perbaikan, saran perbaikan pertama adalah perbaikan check sheet serta perancangan SOP penggunaan check sheet per komponen yang terdapat pada double bottom structure dan wing tank construction, saran perbaikan kedua adalah penandaan daerah las dengan menggunakan cat semprot atau kapur, saran perbaikan ketiga adalah dilakukannya pengawasan terhadap mekanisme perekrutan welder dengan melakukan simulasi pengelasan, meskipun welder tersebut telah memenuhi standar sertifikasi, saran perbaikan keempat adalah dilakukannya pengaturan penggunaan arus pengelasan berdasarkan pada ukuran elektroda yang digunakan dalam proses pengelasan, dan saran kelima adalah kontrol terhadap proses penyimpanan elektroda sehingga dapat meminimalisir peristiwa terjadinya elektroda yang lembab.
English Abstract
This research was conducted at PT. PAL INDONESIA, in the QA and Standardization Department. The object of this research is KM. Pagerungan with a capacity of 17.500 Dead Weight Tonnage (DWT). According to the inspection result from class/Owner Surveyor (OS) on KM. Pagerungan for assembly processing indicated that the largest number of defect existed in the Hull Construction (HC) by as much as 129 defects, followed by Hull Outfitting (HO) 60 defects, Machinery Outfitting (MO) 32 defects, and Electrical Outfitting (EO) 22 defects. Based on these data, it is necessary to analyze the types of defects that have the most influence on the high number of defects in HC to produce applicable solutions. FMEA method is used to produce RPN value for all types of defects that occur in the HC. Defects with the most influential RPN value will be the main focus to be processed by using FTA method. FTA can reveal the cause of the defect types, then suggestions for the improvements in accordance with the cause of the defect types were designed. The result of FMEA method shows that there are three types of defects with the highest RPN value. These are missing bracket with the value of RPN 384, missed weld with the value of RPN 240 and misalignment with the value of RPN 224. These three types of defects became the main focus to be processed by using FTA method. Based on FTA result indicates that these three types of defects have relevance in term of the main causes. Refers to human error and process control which is not optimal. Based on those causes five suggestions for improvement were designed. The first suggestion is check sheet repair and designing SOP based on components in double bottom construction and wing tank construction. The second is marking the weld area by using spray paint or chalk. The third is to supervise the recruitment mechanism of welder by confirm the skill with weld sertification. The forth is controlling weld setting especially for welding current based on the size of electrode. The final suggestion is to control the storage of electrode, so that can minimize due to moist electrode accident.
