Abstract

Penyembuhan luka pada operasi bedah dapat mengalami infeksi akibat penanganan dan kondisi yang kurang tepat. Infeksi nosokomial merupakan infeksi yang berkembang di rumah sakit dan menyerang penderita dalam masa perawatan akibat transmisi berupa mikroba patogen seperti Staphylococcus aureus. S. aureus dapat bersifat multi drug resisten , oleh karena itu dibutuhkan terapi dari bahan alam untuk mempercepat kesempuhan luka. Virgin Coconut Oil (VCO) merupakan bahan alternatif penyembuhan luka karena mengandung asam lemak bersifat antibakteri, mempercepat proliferasi fibroblast dan antiinflamasi. Penelitian ini bertujuan untuk mengetahui efek terapi VCO pada hewan model nosokomial terhadap ekspresi interleukin 8 (IL-8) dan histopatologi jaringan kulit. VCO dibuat dengan pengasasman jeruk nipis 1 %. Sebanyak 20 ekor hewan coba berupa Mus musculus galur BALB/c, jantan, berat 24,2 ± 30,0 gram, umur 8 minggu dibagi menjadi 5 kelompok. Model infeksi nosokomial berupa luka insisi dijahit dengan benang terkontaminasi S. aureus 105 CFU/ml. Kelompok terdiri atas kontrol negatif (dijahit aseptis), kontrol positif (infeksi tanpa terapi), dan 3 kelompok infeksi diterapi dengan perbedaan frekuensi pemberian VCO yaitu P1(1x/hari), P2 (2x/hari) serta P3(3x/hari) selama 7 hari terapi. Variabel penelitian adalah ekspresi IL-8 diukur dengan metode flowcytometry dan dianalisa dengan uji one way ANOVA dilanjutkan uji BNJ (p<0,05), histopatologi jaringan kulit diamati dengan pewarnaan HE yang dianalisa secara deskriptif. Hasil terbaik menunjukan bahwa pemberian VCO 1x/hari mampu menurunkan ekspresi IL-8 tertinggi dan keradangan jaringan kulit luka menurun pada luka insisi model nosokomial. Kesimpulan penelitian ini VCO hasil pengasaman jeruk nipis dapat digunakan sebagai alternatif terapi luka infeksi nosokomial oleh S. aureus

English Abstract

Wound healing in surgical operation can be infection due to improper handling and conditions. Nosocomial infection was an infection that develops in the hospital and was cured during the treatment due to the transmission of pathogenic microbes such as Staphylococcus aureus. S. aureus can be multi-drug resistant, therefore it requires therapy from natural materials to accelerated wound compliance. Virgin Coconut Oil (VCO) was a substitute for wounds because it contains antibacterial, accelerated the proliferation of fibroblasts and anti inflammation. This study aims to determine the effect of VCO therapy on nosocomial model animals on interleukin 8 (IL-8) expression and skin tissue histopathology.VCO was made by 1% acided of lime. A 20 animal models in the form of Mus musculus, BALB / c strain, male, weight 24.2 ± 30.0 grams, age 8 weeks were divided into 5 groups. Model of nosocomial infection were incised wound those sewn with S. aureus 105 CFU / ml contaminated thread. The group consisted of negative control (aseptic sutures), positive control (infection without therapy), and 3 groups of infections treated with differences in the frequency of VCO administration, as P1 (1x / day), P2 (2x / day) and P3 (3x / day) during 7 days of therapy. The research variable were IL-8 expression measured by flowcytometry method and analyzed by one way ANOVA test followed by Tukey test (p <0.05), and skin tissue histopathology was observed with HE was analyzed descriptively. The best results showed that administration VCO 1x / day was able to reduce the highest IL-8 expression and repair in wound skin tissue in nosocomial incision wounds. The conclusion of this study is that VCO results from lime acidification can be used as an alternative therapy for wound nosocomial infections by S. aureus
