Abstract

Rhabdomiolisis merupakan kondisi dimana adanya kerusakan pada jaringan otot yang berakibat terlepasnya komponen serat otot (elektrolit, mioglobin, hemoglobin, dan protein intraseluler) ke darah. Rhabdomiolisis banyak terjadi pada kejadian luka bakar, dimana 50% kasusnya dapat menyebabkan gagal ginjal akut. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh kejadian rhabdomiolisis akibat induksi gliserol terhadap kerusakan histopatologi otot dan kadar Blood Urea Nitrogen (BUN). Hewan model yang digunakan adalah tikus putih (Rattus norvegicus) jantan, umur 14 minggu, berat rata-rata 150 gram. Tikus dibagi menjadi lima kelompok yaitu kelompok I (kontrol negatif) diinduksi dengan aquades steril sebanyak 10 mL/kg BB secara intramuskular, kelompok perlakuan II, III, IV, dan V diinduksi gliserol 50% secara intramuskular sebanyak 10 mL/kg BB. Kelompok II post induksi gliserol jam ke 1, kelompok III pada jam ke 3, kelompok VI pada jam ke 6, kelompok V pada jam ke 12. Parameter BUN diamati dengan metode spektofotometri dan dilanjutkan uji Tukey. Pada otot diamati histopatologi dengan pewarnaan hematoksilin eosin (HE) menggunakan perbesaran 400 kali dengan mikroskop BX-5. Hasil penelitian menunjukkan induksi gliserol secara signifikan (p<0.05) terlihat adanya peningkatan kadar BUN dari kelompok II hingga kelompok V. Perubahan histopatologi meliputi adanya infiltrasi sel radang, nekrosis liquefaktif dan eksudat fibrin pada otot. Kesimpulan dari penelitian ini induksi gliserol 50% dapat meningkatkan kadar BUN dan menyebabkan kerusakan otot untuk menyerupai kejadian rhabdomiolisis.

English Abstract

Rhabdomyolysis was a condition where muscle tissue was damaged which led to the components of skeletal muscle cells (electrolytes, myoglobin, hemoglobin and intracellular protein) released to the blood. Rhabdomyolysis was common on burns and 50% of them could lead to acute kidney failure. The purpose of this research was to find out the effects of rhabdomyolysis caused by glycerol inductions to muscle hyspathologic damage and level of Blood Urea Nitrogen (BUN). The test subjects were white male rats (Rattus norvegicus), aged 14 weeks, average weight of 150 grams. There were five groups of rats, the first (control negative) was inducted by 10mL/kg (of body weight) of sterile aquades intramuscularly, the second, third, fourth and fifth group were inducted by 10mL/kg (of body weight) of 50% glycerol intramuscularly. The second group was analyzed on the first hour post-glycerol induction, the third group on the third hour, the forth on the sixth hour and the fifth group on the twelveth hour. The BUN parameter was analyzed by spectrophotometry and Tukey Test. The muscle hyspathology was analyzed by HE coloring using 400 times magnification on the BX-5 microscope. Research results showed, with significant glycerol induction (p<0.05), there was an increase in levels of BUN from the second to fifth group. Hyspathologic changes included infiltration of inflammatory cells, liquefactive necrosis and fibrin exudate on muscle. The summary of this research was 50% glycerol induction could increase level of BUN and caused muscle damage similar to rhabdomyolysis.
