Abstract
Artritis Reumatoid (AR) merupakan penyakit autoimun yang bersifat inflamasi kronik-sistemik. Inflamasi kronik menyebabkan hipertropi dan penebalan membran synovial. Interleukin-1 beta (IL-1β) merupakan sitokin proinflamatori yang terlibat dalam kerusakan sendi. Buah kesemek memiliki kandungan antioksidan dan antiinflamasi yang dapat digunakan sebagai pengobatan AR. Penelitian ini bertujuan untuk mengetahui pengaruh ekstrak air buah kesemek junggo (Diospyros kaki l.f.) terhadap ekspresi IL-1β dan gambaran histopatologi jaringan sendi tikus (Rattus novergicus) artritis. Penelitian ini mengunakan hewan coba tikus yang dikelompokan menjadi 4 kelompok yaitu kelompok kontrol, kelompok artritis, kelompok artritis terapi 750 mg/Kg BB dan kelompok artritis terapi 1000 mg/Kg BB. Tikus AR dibuat dengan cara injeksi Complete Freund’s Adjuvant (CFA) secara intradermal. Terapi buah kesemek junggo diberikan dengan cara peroral. Ekspresi IL-1β diamati dengan teknik imunohistokimia. Histopatologi sendi diamati secara kualitatif menggunakan mikroskop Olympus BX51. Hasil penelitian menunjukan perbedaan secara signifikan (p<0,05) pemberian CFA menyebabkan inflamasi dengan peningkatan kadar IL-1β nilai rata-rata 350%. Kelompok artritis dengan terapi ekstrak air kesemek junggo pada dosis 750 mg/kg BB menunjukan penurunan ekspresi IL-1β yaitu 70% pemberian dengan dosis 1000 mg/kg BB menunjukan penurunan yang lebih besar yaitu74%. Hasil pengamatan histopatologi terjadi kerusakkan pada membran sinovium pada kelompok artritis, sedangkan kelompok yang mendapat terapi kerusakkan sendi berkurang. Pemberian ekstrak air buah kesemek dapat menyembuhkan artritis reumatoid dilihat dari penurunan ekspresi IL-1β dan perbaikan gambaran histopatologi.
English Abstract
Rheumatoid Arthritis is an autoimmune disease that result in a chronic systemic inflamatory which characterized by hyperthropic and thickening of the synovial membrane. Interleukin 1 beta (IL-1β) is a pro-inflammatory cytokine that mediated joint inflammation and damage. Persimmon fruit (Diospyros feet L.f) has an anti-oxidan and anti-inflammatory substance which can use as a therapy for rheumatoid arthritis. The aim of this research was to discover a persimmon fruit water extrak effect towards IL-1β expression and join histopathology of arthritis rat (rattus novergicus). Four groups of rat were used in this research which were control group, arthritis group, arthritis with 750 mg/kg treatment dose group, and arthritis with 1000 mg/kg treatment dose group. Arthritis rat was mode by intradermal injection of CFA. Persimmon fruit treatment was given by peroral. The level of IL-1β was assessed by immunohistochemistry and analyzed by ANOVA in SPSS 16,0 for Windows. The histopathology of joint was observed qualitatevely by Olympus BX51 microscope. The result showed that IL-1β levels increased significantly (p<0,05) in CFA induced group 350%. While in CFA induced with 750 mg/kg treatment dose group, the IL-1β levels decreased 70%. The decreased of IL-1β level was more significant in CFA induced with 1000 mg/kg treatment dose group 74%. Histopatological feature showed joint damage was decrease in arthritis with treatment group. Based on IL-1β expression descent and repair of histopathology water extract persimmon fruit can be a treatment for rheumatoid arthritis.
