Abstract

Pemakaian alat ortodonti lepasan dalam jangka waktu lama di dalam mulut mempengaruhi kebersihan gigi dan mulut karena dapat menyebabkan akumulasi mikroorganisme pada rongga mulut. Candida albicans merupakan salah satu mikroorganisme yang dapat menimbulkan infeksi oral kandidiasis. Pengaplikasian desinfektan digunakan untuk mencegah akumulasi mikroorganisme. Khlorheksidin merupakan bahan desinfektan golongan bisguanid yang umum digunakan. Siwak diketahui sebagai bahan herbal yang memiliki daya antimikroba dan daya antijamur adekuat. Tujuan penelitian ini adalah untuk mengetahui perbandingan efektivitas ekstrak kayu Siwak (Salvadora persica) dengan klorheksidin terhadap jumlah Candida albicans sebagai desinfektan pada alat ortodonti lepasan. Tiga puluh alat ortodonti lepasan (plat akrilik berupa cengkram adam) direndam dalam saliva buatan selama 1 jam dan suspensi Candida albicans selama 24 jam, kemudian dibagi menjadi tiga kelompok. Kelompok I plat direndam dalam aquades (kontrol), kelompok II direndam dalam klorheksidin 2%, kelompok III direndam dalam ekstrak Siwak 100%. Masing-masing plat dimasukkan dalam aquades steril lalu divibrasi menggunakan vortex mixer selama 30 detik. Pengenceran serial dilakukan dari hasil vibrasi dari masing-masing plat. 0,1ml hasil pengenceran dimasukkan ke dalam Saboraud dextrose agar, dilakukan streaking dan diinkubasi selama 48 jam/37°C. Koloni yang terbentuk dihitung menggunakan colony counter. Data dianalisis menggunakan ANOVA satu jalur dan uji Post-hoc Tukey HSD. Hasil penelitian menunjukkan bahwa terdapat perbedaan jumlah koloni Candida albicans yang signifikan antara ketiga kelompok (p=0,00). Uji Post hoc Tukey HSD dan persentase penurunan jumlah Candida albicans menunjukkan bahwa terdapat perbedaan rerata jumlah Candida albicans yang bermakna antara kedua desinfektan, dimana klorheksidin memiliki persentase penurunan Candida albicans lebih tinggi dibandingkan ekstrak Siwak. Kesimpulan penelitian ini adalah efektivitas klorheksidin 2% lebih besar sebagai desinfektan alat ortodonti lepasan dibandingkan ekstrak Siwak 100% dalam menurunkan jumlah Candida albicans.

English Abstract

The use of a removable orthodontic appliance for a long time in the mouth affects oral hygiene because it can lead to the accumulation of microorganisms in the oral cavity. Candida albicans is microorganism that can cause oral candidiasis infection. Disinfectant was applied to prevent the accumulation of microorganisms. Chlorhexidine is a disinfectant bisguanide group that has been commonly used. Siwak was considered as herbal ingredients that have an adequate antimicrobial activity and antifungal activity. The aim of this research was to find out the comparison in effectiveness of 100% Siwak (Salvadora persica) extract with 2% chlorhexidine to against Candida albicans number as a disinfectant on removable orthodontic appliance. Thirty removable orthodontic Appliance (acrylic plates with adam claps) were immersed in artificial saliva for 1 hour and suspensions Candida albicans for 24 hours, and divided into three groups afterwards. First group was soaked in distilled water (control), second group was soaked in 2% chlorhexidine, and third group was soaked in 100% Siwak extract. Each plates was inserted in sterile distilled water and shaked using a vortex mixer for 30 second. Dilution series was performed from the results of each plate shaking. 0,1ml of dilution series was put on Saboraud dextrose agar, streaking and incubated for 48 hours/37°C. Performed colonies was calculated using a colony counter. Data were analyzed using oneway ANOVA, followed by Post hoc Tukey HSD tests. The results showed that there were significantly differences in the number of colonies of Candida albicans among three groups (p =0,00). Post hoc Tukey HSD test and the decreasing percentage of Candida albicans numbers showed that there were significant differences mean of Candida albicans between the two disinfectants, in which the decreasing percentage of Candida albicans using chlorhexidine was higher compared to Siwak extract. It was concluded that the effectiveness of 2% chlorhexidine as disinfectant in removable orthodontic appliance greater than 100% Siwak extract on the growth of Candida albicans.
