Abstract
Sepsis neonatorum mendapati tingkat pertama penyebab mortalitas pada neonatus di Indonesia. Diagnosis sepsis menjadi sulit ditegakkan karena diagnosis klinis yang tidak spesifik dan kultur darah yang lama. Pada keadaan sepsis, menyebabkan berkurangnya kadar hemoglobin pada eritrosit sehingga memicu terjadinya anemia yang berpengaruh negatif terhadap penghantaran oksigen ke jaringan. Indeks eritrosit terdiri dari MCV, MCH, MCHC, dan RDW yang merupakan nilai penting untuk menetapkan kelainan anemia secara morfologis. Penelitian ini menggunakan metode cross sectional yang bertujuan untuk mengetahui nilai diagnostik dan prognostik dari hemoglobin dan indeks eritrosit pada sepsis neonatorum dengan mengambil data sekunder di rekam medik Rumah Sakit Umum Dr. Saiful Anwar Malang. Subyek penelitian adalah neonatus yang diduga menderita sepsis neonatorum. Subyek yang didapat adalah 50 subyek. Nilai diagnostik yang didapatkan yaitu sensitivitas dan spesifisitas untuk kadar hemoglobin dengan cut off < 11,7 g/dl yaitu 53% dan 70 % ; eritrosit dengan nilai cut off ≤ 3,97 μL yaitu 63% dan 40% ; dan RDW dengan cut off >16,5% yaitu 56 % dan 45%. Sedangkan untuk nilai prognostik yang didapatkan dari spesivisitas dan sensitifitas untuk kadar hemoglobin dengan cut off <11,7 g/dl yaitu 58,3% dan 44,7% ; eritrosit dengan cut off < 3,85 μL yaitu 67% dan 50% ; RDW dengan cut off <16,5 % yaitu 58% dan 53%. Kesimpulan penelitian ini adalah nilai diagnostik eritrosit dengan nilai cut off ≤ 3,97 μ μL dan nilai prognostik eritrosit dengan nilai cut off <3,85 μL merupakan marker paling sensitif untuk diagnostik dan prognostik sepsis neonatorum dibandingkan marker penelitian lainnya.
English Abstract
Neonatal sepsis obtains the first rate of neonatal mortality causes in Indonesia. The diagnosis turns to be difficult due to unspecific clinical diagnosis and long duration of blood culture. In the condition of sepsis, it causes declined haemoglobin concentration on erythrocytes, therefore triggers the occurrence of anemia which affects negatively towards delivering oxygen to tissue. Erythrocytes index consists of MCV, MCH, MCHC, and RDW that are important values for specifying anemia disorder morphologically. This study used cross sectional method that was intended to determine the diagnostic and prognostic value from haemoglobin and erythrocyte index on neonatal sepsis by collecting secondary data on medical records in Rumah Sakit Umum Dr. Saiful Anwar Malang. The study subject was suspected neonates suffering from neonatal sepsis. Subjects obtained were 50 subjects. Diagnostic values obtained were sensitivity and specificity for haemoglobin concentration of sepsis group with cutoff of < 11,7 g was 53% and 70 %; erythrocytes with cutoff of < 3,97 μL was 63% and 40% ; and RDW with cutoff of >16,5% was 56 % and 45 %. Meanwhile, for prognostic values obtained from specificity and sensitivity of haemoglobin concentration with cutoff of <11, 7 g/dL was 58,3% and 44,7% ; erythrocytes with cutoff of < 3, 85 μL was 67 % and 50 % ; RDW with cutoff of <16,5 % was 58% and 53%. The conclusion of this study is erythrocyte diagnostic value with cutoff value of ≤ 3,97 μL and erythrocyte prognostic value with cutoff value of <3,85 μL is the most sensitive marker for diagnostic and prognostic of neonatal sepsis compared to other study markers.
