Abstract
Gout Arthritis atau yang lebih dikenal sebagai penyakit asam urat atau gout, merupakan penyakit sendi yang diakibatkan oleh timbunan kristal asam urat. Penderita gout memiliki kecenderungan untuk memiliki Health-Related Quality of Life yang buruk dan komorbiditas penyakit metabolik. Salah satu faktor risiko yang diketahui berpengaruh adalah obesitas. Penelitian ini bertujuan untuk mengetahui hubungan antara salah satu metode penentu obesitas yaitu Body Mass Index (BMI) dengan kejadian gout pada masyarakat kota Malang. Jenis penelitian ini adalah observasional dengan pendekatan cross-sectional yang dilakukan dengan metode survei menggunakan kuisioner yang diadaptasi dari WHO-ILARCOPCORD. Sampel dalam penelitian ini sejumlah 2067 masyarakat kota Malang yang dipilih melalui multi stage random sampling. Penelitian ini membandingkan dua kelompok populasi yaitu populasi dengan BMI < 23 kg/m2 dan populasi dengan BMI ≥ 23 kg/m2. Berdasarkan hasil penelitian, didapatkan prevalensi gout di kota Malang sebanyak 0,73%, dengan sebanyak 1% dari subjek dengan BMI ≥ 23 kg/m2 (n = 1294) dan 0,3% dari subjek dengan BMI < 23 kg/m2 (n = 773) menderita gout. Hasil analisis hubungan antara BMI dengan gout menggunakan chi-square menunjukkan tidak adanya hubungan signifikan antara kedua variabel tersebut (p = 0,053, α = 0,05) dengan nilai OR = 3,912 (CI 95% = 0,880 - 17,383). Kesimpulan penelitian ini adalah tidak terdapat hubungan antara BMI dengan gout pada masyarakat Kota Malang yang dapat terjadi karena faktor kebiasaan diet purin dan genetik dari masyarakat tersebut serta jenis obesitas yang diduga berhubungan dengan kejadian gout adalah obesitas viseral, yang tidak dapat ditentukan berdasarkan BMI saja.
English Abstract
Gout Arthritis, known as uric acid disease or gout, is a joint inflammatory disease which is caused by uric crystal deposits. Gout patients have tendency to develop poor health-related quality of life and metabolic diseases comorbidity. One of the known risk factor of gout is obesity. This study aimed to determine the association between one of the measurement methods of obesity, the Body Mass Index (BMI), and the incidence of gout in Malang. This cross-sectional observational study was conducted by survey, using instrument adapted from WHO-ILAR-COPCORD questionnaire. A number of 2067 respondents were chosen through multi stage random sampling. This study compared two groups of populations, which were a population with BMI < 23 kg/m2 and a population with BMI ≥ 23 kg/m2. Based on the study, the prevalence of gout in Malang was 0.73%, specifically 1% among subjects with BMI ≥ 23 kg/m2 (n = 1294) and 0.3% among subjects with BMI < 23 kg/m2 (n = 773) diagnosed as gout. The statistical analysis of the relationship between BMI and gout using chi-square test showed no significant association (p = 0.053, α = 0.05) with OR = 3.912 (95% CI = 0.880 to 17.383). It is concluded that there is no association between BMI and gout in Malang population that may be caused by the influence of dietary purin intake and genetic factors in the population and the type of obesity that is suspected to have association with gout is visceral obesity, that can not be assessed using BMI only.
