Abstract

Artritis gout adalah peradangan sendi akibat deposit monosodium urat dalam cairan sinovial. Serangan arthritis gout akut ditandai dengan nyeri, bengkak kemerahan, hangat, dan infiltrasi sel polimorfonuklear/PMN (neutrofil) di sinovium. Kondisi gout yang berlanjut kronis dapat menyebabkan malformasi sendi sehingga gout perlu ditangani sejak akut. Secara in vitro dilaporkan bahwa ApoB (Apolipoprotein B100) dapat mengurangi respon inflamasi terhadap kristal asam urat, tetapi penelitian in vivo belum dilakukan. Penelitian ini bertujuan untuk mengetahui apakah ApoB dapat menurunkan jumlah PMN dalam hewan model artritis gout akut. Penelitian In vivo true experimental and randomized posttest only controlled group dilakukan pada Rattus norvegicus berusia 18-20 minggu. Inflamasi gout akut diinduksi menggunakan 0,25/50μl kristal monosodium urat (MSU) pada sendi genu. Penelitian ini menggunakan lima kelompok tikus artritis gout akut dengan lima tikus per kelompok. Kelompok I adalah kelompok kontrol positif dan kelompok II adalah kelompok pembanding yang diterapi kolkisin. Kelompok III, IV, V masing-masing diinjeksi intra artikular ApoB 1,25 μg, 2,5 μg, dan 3,75 μg. Enam jam setelah injeksi ApoB, jaringan sinovium sendi genu diambil untuk pemeriksaan histologis. Sel PMN diperiksa dengan 1000x perbesaran dan 8 lapangan pandang untuk setiap tikus. Pemberian ApoB 3,75 μg menunjukkan penurunan signifikan dalam jumlah sel PMN dibandingkan dengan kelompok yang diterapi kolkisin (LSD, p = 0,001). Hubungan dosis ApoB dengan jumlah sel PMN juga signifikan (uji regresi linier, p = 0,000, korelasi: -0,833). Injeksi intra-artikular ApoB dapat menurunkan jumlah sel PMN di sinovium pada hewan model artritis gout akut. Injeksi intra-artikular ApoB berpotensi sebagai terapi artritis gout akut.

English Abstract

Gouty arthritis is an inflammation of joint induced by deposit of monosodium urate in synovial fluid. Acute gouty arthritis attack is characterized by painful red warm swollen joint and infiltration of polymorphonuclear cells (neutrophils) to synovium. If gout continue to chronic state it can cause joint malformation. Thus gout need to be treated while in acute condition. In vitro studies found that ApoB (Apolipoprotein B100) can reduce inflammation response against uric acid crystal, but in vivo study haven’t been conducted. This research aimed to know if ApoB can decrease the number of neutrophils in acute gouty arthritis animal model. In vivo true experimental and randomized posttest only controlled group research was conducted in 18-20 weeks old Rattus norvegicus. Acute gouty inflammation was induced using 0.25mg/50μl monosodium urate (MSU) crystal on dextral genu. Five groups of acute gouty arthritis rats with 5 rats per groups were used for the study. Groups I and II served as non-treated gouty and gouty controls receiving oral colchicine therapy respectively. Groups III, IV, V received intra articular ApoB 1.25 μg, 2.5 μg, and 3.75 μg respectively. 6 hours after injection of ApoB, dextral genu synovium tissues were collected for histopathology anatomy examination. PMN cells were examined in 1000x zoom and 8 observation field for each rat. Administration of 3.75 μg ApoB showed significant reduction in number of PMN cells compared to colchicine treated groups (LSD, p=0.001). There were also significant correlation between doses of ApoB given and number of PMN cells (Linier Regression test, p=0.000, Correlation: -0.833). We conclude that intra articular injection of ApoB can decrease the number of PMN cells in synovium of acute gouty arthritis animal model. This research suggests that intra articular injection of ApoB hold potency of treatment for acute gouty arthritis.
