Pengaruh Ekstrak Kulit Tomat (Solanum Lycopersicum) Terhadap Sensitivitas Insulin Pada Tikus Model Diabetes Melitus Tipe 2

Abstract

Diabetes melitus tipe 2 (DM tipe 2) merupakan gangguan metabolik akibat penurunan sensitivitas insulin yang disebabkan berkurangnya sekresi atau penurunan sensitivitas jaringan terhadap insulin. Resistensi insulin pada DM tipe 2 sering dikaitkan dengan terganggunya sensitivitas jaringan terhadap insulin, sehingga menyebabkan hiperglikemia dan peningkatan radikal bebas. Dibutuhkan antioksidan untuk dapat menetralkan radikal bebas pada DM tipe 2. Tomat (Solanum lycopersicum) mengandung beberapa antioksidan seperti β karoten, likopen, lutein, β cryptoxanthin, vitamin A, vitamin C, dan vitamin E. Likopen merupakan antioksidan tertinggi di dalam kulit tomat yang mampu memperbaiki gangguan fungsi sel β pankreas dan meningkatkan sensitivitas insulin pada DM tipe 2. Tujuan penelitian ini adalah untuk mengetahui pengaruh ekstrak kulit tomat (Solanum lycopersicum) terhadap sensitivitas insulin pada tikus (Rattus norvegicus) galur wistar jantan dengan DM Tipe 2. Penelitian ini menggunakan Randomized Post Test Controlled Group Design dengan teknik sampling Simple Randomized Sampling. Sampel dibagi menjadi lima kelompok, yaitu kontrol negatif (n=4), kontrol positif (n=4), kelompok perlakuan 1 yang diberi ekstrak kulit tomat dosis 50mg/kgBB (n=4), kelompok perlakuan 2 dengan dosis 100 mg/kgBB (n=4), dan kelompok perlakuan 3 dengan dosis 150 mg/kgBB (n=4). Terapi ekstrak kulit tomat diberikan selama 4 minggu dan kemudian kadar glukosa darah dan insulin diukur untuk mendapatkan indeks sensitivitas insulin. Indeks sensitivitas insulin diuji statistik One Way Anova dengan signifikansi 0.05 dan didapatkan hasil yang signifikan (P = 0.037). Hasil uji Post-Hoc terdapat perbedaan yang signifikan antara kelompok KN dan KP, serta tidak ada perbedaan yang signifikan pada kelompok yang diberi ekstrak kulit tomat dengan kelompok KN dan KP. Kesimpulan dari penelitian ini adalah terdapat penurunan sensitivitas insulin yang signifikan pada tikus model DM tipe 2 dibandingkan dengan tikus normal dan kelompok yang diterapi ekstrak kulit tomat memiliki sensitivitas insulin yang lebih tinggi dibandingkan KP, namun secara statistik tidak signifikan.

English Abstract

Diabetes mellitus type 2 (DM type 2) is a metabolic disorder due to the decrease of insulin sensitivity caused by the decrease of insulin secretion or the decrease of tissue sensitivity to insulin. Insulin resistance in DM type 2 is often associated with disruption of tissue sensitivity to insulin that induce hyperglycemia and increasing of free radicals. Antioxidants were needed to neutralized free radicals in DM type 2. Tomato (Solanum lycopersicum) contains of several antioxidant, such as β-carotene, lycopene, lutein, cryptoxanthin β, vitamin A, vitamin C, and vitamin E. Lycopene is the highest antioxidant in tomato peel, resulting the improvement of sel β pancreas function and increased insulin sensitivity in DM type 2. The goal of this research to determined the effect of tomato peel extract toward insulin sensitivity in rats with DM type 2. This research used Randomized Post Test Controlled Group Design with Simple Randomized Sampling technique. Sample were divided into five groups: negative control (n=4), positive control (n=4), three treatment groups were given tomato peel extract with dosage 50mg/kgBW (n=4), group 2 with dosage 100 mg/kgBW (n=4), and group 3 with dosage 150 mg / kg (n = 4). Tomato peel extract therapy were for 4 weeks and then blood glucose and insulin levels were measured to measured insulin sensitivity index. The result of insulin sensitivity index in One Way Anova statistical test with a significance of 0.05 showed a significant result (P = 0.037). The result of Post-Hoc test, there was a significant differences between KN and KP group and there was no significantly differences between group that given tomato peel extract with KN and KP groups. The conclusion of this research is there is an increasing of insulin sensitivity in rats with DM type 2 that compare with normal rats and group that treated with tomato peel extract had a higher insulin sensitivity than KP, but not statistically significant.
