Abstract

Penyakit hati kronis merupakan salah satu masalah serius dibidang kesehatan. Fibrosis hati merupakan respon penyembuhan luka terhadap jejas hati kronik berupa terbentuknya jaringan ikat dan akumulasi dari protein matriks ekstraseluler yang akan merusak arsitektur hati. Fibrosis hati akibat karbon tetraklorida (CCl4) menyebabkan kerusakan pada hati, ginjal, paru, dan susunan saraf pusat. Jejas yang terjadi secara terus menerus mengaktivasi berbagai sitokin proinflamasi yaitu Interleukin-17 (IL-17) sebagai hasil sekresi dari subset baru famili sel T CD4+, yaitu Th 17, yang telah dibuktikan dalam berbagai penelitian memiliki kontribusi yang besar pada kejadiaan fibrosis di hati. IL-17 di hati dapat memicu sekresi sitokin inflamasi lainnya, dan memicu Hepatic Stellate Cell (HSC) untuk beraktivasi dan berubah menjadi myofibrogenic-like cell yang memicu terjadinya fibrosis hati. Regresi spontan akan terjadi ketika paparan zat hepatotoksik dihentikan. Kurkumin sebagai salah satu antifibrotik yang telah terbukti pada berbagai penelitian dibidang gastrointestinal akan melemahkan stress oksidatif, menekan inflamasi pada tikus dengan ekspresi sitokin pro inflamasi yang menghambat aktivasi HSC serta meningkatkan apoptosis. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian kurkumin terhadap penurunan kadar IL-17 serum pada tikus model fibrosis yang diinduksi karbon tetraklorida (CCl4). Penelitian ini merupakan penelitian eksperimental dengan metode rancangan acak lengkap (RAL). Hewan coba dibagi menjadi delapan kelompok perlakuan dengan waktu paparan CCl4 selama 9 minggu kemudian dilanjutkan dengan pemberian kurkumin secara intraperitonial pada kelompok KP dan pemberian pelarut kurkumin pada kelompok KK dengan lama pemberian yang sama selama 2, 5, dan 9 minggu. Diakhir perlakuan, IL-17 serum diukur menggunakan metode Enzyme-linked Immunosorbent Assay (ELISA). Sehingga hasil dari penelitian ini, dinyatakan bahwa pemberian kurkumin lebih menurunkan kadar IL-17 serum dibanding tanpa pemberian kurkumin jika diberikan selama 5 minggu. Hal ini ditunjukkan dengan uji beda Tuckey didapatkan kelompok yang berbeda secara bermakna antara kelompok K-Positif dengan kelompok KP-5 (p = 0,007; p < 0,05).

English Abstract

Chronic liver disease is one of the serious problems in medical aspect. Liver fibrosis is a wound healing response to chronic liver injury such as connective tissue formation and accumulation of extracellular matrix proteins that will damage the liver architecture. Fibrosis of the liver caused by carbon tetrachloride (CCl4) cause damage to the liver, kidneys, lungs, and central nervous system. The lesion that occurs continuously activates proinflammatory cytokines, namely interleukin-17 (IL-17) as a result of the secretion of a subset of new families of CD4 + T cells: Th 17, which has been demonstrated in numerous studies to have a great contribution to the occurrence of fibrosis in the liver. IL-17 in the liver can trigger other inflammatory cytokine secretion, and triggers Hepatic stellate cell (HSC) activated and turned into myofibrogenic-like cell that trigger the occurrence of liver fibrosis. Spontaneous regression will occur when exposure to hepatotoxic agents stopped. Antifibrotik curcumin as one that has been proven in many studies in the field of gastrointestinal will weaken oxidative stress, suppress inflammation in mice with the expression of pro-inflammatory cytokines that inhibit activation of HSC and increase apoptosis. This study aimed to determine the effect of curcumin on the decrease in serum levels of IL-17 in mice model of fibrosis induced by carbon tetrachloride (CCl4). This study is an experimental method completely randomized design (CRD). Animals were divided into eight groups treated with CCl4 exposure time for 9 weeks and then followed by intraperitoneal administration of curcumin in the KP group and solvent administration of curcumin on a group of families with a duration of the same for 2, 5, and 9 weeks. At the end of the treatment, IL-17 serum were measured using enzyme-linked immunosorbent assay (ELISA). Thus, from the result of this study, it is stated that giving curcumin further lowered serum levels of IL-17 than without curcumin over a time span of 5 weeks. This is shown by the different test groups Tuckey obtained significantly different between groups K-positives with KP-5 group (p = 0.007; p <0.05).
