Abstract

Kanker kolorektal merupakan keganasan usus besar dengan patogenesis yang kompleks dan multifaktorial. Resiko kanker kolorektal lebih tinggi pada pasien dengan kolitis kronik. Proses inflamasi kronis pada lapisan mukosa kolon menyebabkan migrasi limfosit melalui monocyte chemoattractant protein 1 (MCP-1) untuk menarget sel yang mengalami transformasi dan mengaktivasi makrofag agar segera mengeliminasi tumor sebelum menghasilkan manifestasi klinis yang bermakna. Sekresi IL-10 oleh sel neoplasma dan trans-signaling IL-6 yang menginduksi gen anti-apoptosis Bcl-xl maupun Bcl-2 menyebabkan limfosit akan terakumulasi di jaringan, mengeluarkan sitokin proinflamasi dan memperparah progresifitas tumor. Penelitian ini bertujuan untuk menguji efek ekstrak etanol daun benalu mangga (Dendrophtoe pentandra) dengan kandungan utama diduga flavonoid kuersetin dalam menurunkan infiltrasi limfosit pada kolon mencit model colitis-associated colon cancer dengan induksi AOM dan DSS. Kuersetin bekerja menghambat COX-2 pada proses inflamasi dan menghambat ekspresi MCP-1 sehingga diharapkan mengurangi infiltrasi sel limfosit pada kolon. Pada minggu ke-0 mencit diinduksi AOM secara intraperitoneal, dilanjutkan dengan DSS 5% pada air minum pada minggu 1,3,5,7,9,11, dan 13. Pada minggu ke-6 hingga 14, 3 kelompok perlakuan diberi dosis ekstrak 125 mg/kgBB, 250 mg/kgBB, dan 500 mg/kgBB menggunakan sonde. Pengamatan histopatologi dilakukan dengan menghitung infiltrasi sel limfosit pada mukosa kolon mencit dengan pewarnaan HE. Hasil ANOVA menunjukkan penurunan yang signifikan (p=0.000, p<0.05) setelah perlakuan dengan korelasi pearson bernilai negatif (-0.977). Dapat disimpulkan, pemberian ekstrak etanol daun benalu mangga (Dendrophtoe pentandra) dapat menurunkan jumlah infiltrasi sel limfosit pada kolon mencit model colitis-associated colon cancer, dengan dosis optimal 500 mg/kgBB.

English Abstract

Colorectal cancer is a malignancy of the large intestine with complex and multi-factorial pathogenesis. Its risk is higher in patients with chronic colitis. Chronic inflammation in colon mucosa layer causes lymphocyte migration through monocyte chemo-attractant protein 1 (MCP-1), functioned to target and destroy transformed cell and activate macrophage before causing significant clinical manifestation. Secretion of IL-10 by neoplasm cell and trans-signlaing of IL-6 induced anti-apoptotic gene bcl-xl and bcl-2 causes lymphocyte to accumulate in tissue, secret proinflammatory cytokines and exacerbate the progression of tumors. This study aimed to examine the effects of ethanol extract of mango leaves (Dendrophtoe pentandra) with suspected main content flavonoid quercetin in reducing infiltration of lymphocytes in the colon of mice model of colitis-associated colon cancer with induction of AOM and DSS. Quercetin inhibits COX-2 in inflammation process and MCP-1 expression which is expected to reduce the infiltration of lymphocytes in the colon. At week 0, AOM is injected intraperitoneally, followed by 5% DSS in drinking water at week 1,3,5,7,9,11 and 13. At week 6 to 14, 3 treatment groups were given doses of extract 125 mg/kgbw, 250 mg/kgbw, 500 mg/kgbw. Histopathological observations done by calculating infiltration of lymphocyte in colonic mucosa of mice with HE staining. ANOVA results showed reduction in lymphocyte infiltration significantly (p=0.000, p<0.05) after treatment and negative Pearson’s correlation (r=-0.977). It can be concluded that ethanol extract of mango leaves (Dendrophtoe pentandra) can reduce the amount of lymphocytes infiltrated in the colon of colitis-associated colon cancer mice model, with an optimal dose 500 mg/kgbw.
