Abstract

Tuberkulosis merupakan penyakit infeksi yang disebabkan oleh bakteri bentuk batang (basil) dengan nama Mycobacterium tuberculosis. Bakteri tuberkulosis ditularkan melalui air ludah atau dahak penderita yang mengandung bakteri tuberkulosis. Orang sehat yang menghirup udara yang terkontaminasi dahak dari penderita dengan bakteri tuberkulosis kemudian akan jatuh sakit. Bakteri tuberkulosis kemudian akan membentuk fokus primer/fokus Ghon pada paru-paru orang tersebut. Fokus primer nantinya dapat pecah dan masuk ke peredaran darah dan/atau limfe ke seluruh tubuh. Bila bakteri dapat bertahan hidup, bakteri tuberkulosis bisa masuk ke sistem saraf pusat terutama otak. Infeksi tuberkulosis di otak dapat menyebabkan mikroglia mengeluarkan faktor-faktor inflamasi seperti Tumor Necrosis Factor alpha (TNF-α) dan Interleukin 1 beta (IL-1β) muncul sebagai respon imun tubuh. Faktor-faktor inflamasi tadi nantinya dapat memicu mikroglia untuk mengeluarkan Matrix Metalloproteinase-2 (MMP-2) agar dapat meregenerasi sel yang nekrosis atau apoptosis karena inflamasi. MMP-2 terbukti memiliki peranan penting dalam infeksi tuberkulosis di otak. Oleh karena itu, penelitian ini dilakukan untuk mengetahui bagaimana ekspresi MMP-2 pada jaringan otak mencit yang tidak terinfeksi tuberkulosis, terinfeksi tuberkulosis dengan masa inkubasi 8 minggu, dan 16 minggu. Penelitian ini bersifat semikuantitatif dengan membandingkan jumlah ekspresi MMP-2 pada 3 kelompok sampel. Pengamatan ekspresi MMP-2 pada jaringan sel otak mencit dilakukan dengan metode pewarnaan imunohistokimia yang dilihat pada mikroskop dengan perbesaran 400x. Sel otak yang mengekspresikan MMP-2 akan berwarna coklat pada inti, sitoplasma, dan dinding sel. Hasil yang diperoleh adalah ekspresi MMP-2 semakin menurun apabila masa inkubasi infeksi tuberkulosis semakin lama. Namun, ekspresi MMP-2 tidak menurun setelah masa inkubasi bakteri tuberkulosis lebih dari 8 minggu.

English Abstract

Tuberculous infection is one of infectious diseases which caused by basil-type bacteria. Bacteria which cause tuberculosis is Mycobacterium tuberculosis. Mycobacterium tuberculosis spread from one person to another via patient saliva and sputum which contains mycobacterium tuberculosis. Healthy person who breath the air which has been contaminated with this sputum or saliva will become sick. In lungs, mycobacterium tuberculosis will form primary focus / Ghon focus. This primary focus sometimes can rupture and make mycobacterium tuberculosis inside the focus enter via blood or lymph vessels to whole body. If the bacteria manage to survive, mycobacterium tuberculosis will enter center nervous system especially brain. Tuberculous infection in brain can cause microglia to secrete inflammatory factors like Tumor Necrosis Factor alpha (TNF-α) and Interleukin 1 beta (IL-1β) which will be shown as body immune respons. Those inflammatory factors eventually can trigger microglia to secrete Matrix Metalloproteinase-2 (MMP-2) which will regenerate necrotic or apoptosis cells because of inflammation process. MMP-2 has been proven to have important role in brain tuberculous infection. Because of that, this research was conducted to ascertain MMP-2 expression in mus musculus brain tissue with no infection, infection for 8 weeks, and infection for 16 weeks. This research used semiquantitative method to compare MMP-2 expression in 3 samples group. Observation of MMP-2 expression in mus musculus brain tissue were made by using immunohistochemistry colouration method which then would be observed in microscope with 400x magnification. Brain cell which express MMP-2 will become brown in cell nucleus, cytoplasm, and wall. The result which had be obtained was overtime reduction of MMP-2 expression. However, MMP-2 expression didn’t decrease after 8 weeks time of infection.
