Abstract
Osteoartritis (OA) merupakan penyakit radang sendi yang disebabkan proses mekanik seperti trauma dan beban berat serta inflamasi yang berlebihan sehingga mengakibatkan ketidakstabilan proses degradasi dan sintesis pada kartilago sendi yang menyebabkan nyeri sendi kronis dan gangguan fungsi sendi. Sargassum sp, mengandung fucoidan yang mampu meningkatkan mobilisasi mesenchymal stem cells (MSCs) pada area tubuh yang mengalami kerusakan jaringan, mencegah degradasi, serta meningkatkan sintesis asam hialuronat sebagai pelumas alami sendi dalam mencegah gesekan. Tujuan penelitian ini untuk mengetahui pengaruh injeksi fucoidan secara intraartikular terhadap perbaikan histopatologi jaringan sendi tikus model osteoartritis. Penelitian ini menggunakan tikus Rattus norvegicus yang dibagi menjadi 9 kelompok. Kelompok kontrol positif dan perlakuan diinduksi osteoartritis dengan 3 kali injeksi complete freund’s adjuvant (CFA) tiap 2 minggu. Kelompok perlakuan 1, 2 dan 3 diterapi dengan ekstrak fucoidan murni 20, 40 dan 80 mg/kgBB, perlakuan 4 diterapi dengan steroid 10 mg/kgBB, serta perlakuan 5, 6 dan 7 diterapi dengan kombinasi steroid dengan fucoidan sesuai dosis yang telah disebutkan sebanyak 2 kali dengan interval 1 minggu. Pemeriksaan histopatologis jaringan kartilago sendi menunjukkan pemberian terapi fucoidan dosis II (40 mg/kgBB) mampu memperbaiki kerusakan sendi dan mencegah degradasi sendi secara signifikan (p<0,05). Respon terapi terhadap fucoidan menunjukkan hasil yang lebih baik dibandingkan penggunaan steroid injeksi yang biasa digunakan sebagai terapi pada osteoartritis. Dengan demikian, injeksi fucoidan secara intraartikular memiliki potensi sebagai alternatif terapi osteoartritis yang efektif.
English Abstract
Osteoarthritis (OA) is an inflammatory disease of the joints caused by mechanical processes such as trauma, heavy loads and excessive inflammation resulting in instability of degradation and synthesis process in joint cartilage that causes chronic joint pain and dysfunction. Sargassum sp. is containing fucoidan that promote mobilization of mesenchymal stem cells (MSCs) to damaged areas of the body tissue, prevents degradation and increase the synthesis of hyaluronic acid as a natural lubricant to prevent joint’s friction. This study aim to determine the effect of intra-articular injection of fucoidan on the articular bone tissue repair in rat. Animal models of this study divided into 9 groups. Positive control and treatment group induced by complete freund’s adjuvant (CFA) injection 3 times every 2 weeks. Treatment groups 1, 2 and 3 were treated with fucoidan extracts 20, 40 and 80 mg/kg BW, group 4 treated with steroid 10 mg/kg BW, and group 5, 6 and 7 were treated with a combination of steroid and fucoidan 2 times with 1 week interval. Histopathological examination of the articular cartilage tissue showed fucoidan therapy (40 mg/kg BW) is able to repair joint damage and prevent joint degradation significantly (p<0.05). Response to fucoidan therapy showed better results than the use of steroid injections that commonly used in the treatment of osteoarthritis. Thus, the intra-articular injection of fucoidan has potential as an effective alternative treatment of osteoarthritis.
