Abstract
Pada kehamilan trimester pertama kebutuhan zat besi umumnya meningkat. Pada trimester kedua terjadi penambahan volume plasma darah yang tidak sebanding dengan penambahan massa sel darah merah, sehingga terjadi pengenceran darah. Penambahan volume plasma darah pada ibu hamil dapat mencapai 30–50%, sedangkan peningkatan massa sel darah merah hanya 18-25%. Akibatnya, terjadi penurunan kadar hemoglobin dalam darah. Selain kebutuhan zat besi dan pengenceran darah ada faktor lain yang mempengaruhi kadar hemoglobin pada saat hamil, diantaranya faktor sosial dan paritas. Tujuan dari penelitian ini adalah untuk mengetahui hubungan faktor sosial dan paritas terhadap kadar hemoglobin pada ibu hamil trimester pertama dan kedua di Puskesmas Kesamben Kabupaten Blitar. Metode penelitian ini adalah observasional analitik kuantitatif dengan rancangan cohort. Penelitian ini dilaksanakan pada tanggal 26 Februari sampai tanggal 10 Mei 2014 di Puskesmas Kesamben Kabupaten Blitar dengan jumlah sampel 34 ibu hamil yang didapatkan dengan metode quota samping, dimana sampel ditentukan dari banyaknya ibu hamil yang periksa saat trimester pertama di bulan Januari dan Februari berapapun jumlahnya, akan tetapi diharapkan mencapai 30 sampel. Sebagai variabel tergantung adalah kadar hemoglobin dan sebagai variabel bebas adalah ibu hamil. Berdasarkan hasil penelitian didapatkan rata-rata kadar hemoglobin ibu hamil trimester pertama adalah 11,40 g% dan rata-rata pada saat trimester dua adalah 11,06 g% dengan hasil uji t-test diperoleh nilai P = 0,000. Berdasarkan hasil uji chi-square didapatkan hubungan yang bermakna antara pendidikan (p = 0,000), pekerjaan (p = 0,357), penghasilan keluarga perbulan (p = 0,019), paritas (p = 0,023). Yang berart Ho ditolak, sehingga dapat disimpulkan bahwa terdapat hubungan faktor sosial dan paritas terhadap kadar hemoglobin pada ibu hamil trimester pertama dan kedua di Puskesmas Kesamben Kabupaten Blitar.
English Abstract
Generally, in the first trimester of pregnancy, the need of iron in the body was increase. In the second trimester, the addition of blood plasma volume disproportionate with the addition of red blood cell mass, resulting in dilution of blood. Blood plasma volume expansion in pregnant women can reach 30-50%, while the increase in red blood cell mass is only 18-25%. Consequently, decreasing haemoglobin level in the blood. Social factor and parity were other factors that influenced the amount of hemoglobin during pregnancy be sides of zinc and hemodilusion. The aim of this study was to find out the relationship between social factor and parity to amount of hemoglobin on first and second trimester pregnant women at Kesamben Health Center of Blitar Regency. This study used the observational analytic quantitative with cohort. The study was held on 26 February until 10 May, 2014 in Kesamben Health Center of Blitar Regency. The subjects of this study were 34 pregnant women who were obtained using quota sampling method. The subjects were determined from the number of pregnant women during their first trimester pregnancy who checked in January and February, expected number reached 30 samples. As the dependent variable is the level of hemoglobin and as an independent variable is pregnant women. Based on the research result obtained by mean the levels of hemoglobin on first trimester was 11.40 g % and the mean on second trimester was 11.06 g% with t-test obtained P = 0.000. Statistically with using chi-square test had a meaningful relationship between educational (p = 0.000), work (p = 0.357), per month of family ultimate (p = 0.019), parity (p = 0.023). Which means that Ho was rejected, so it can be concluded there was a relationship between social factor and parity to amount of hemoglobin on first and second trimester pregnant women at Kesamben Health Center of Blitar Regency.
