Abstract

Latar Belakang: Daun salam (Eugenia polyantha) dan daun sirih merah (Piper crocatum) secara ethnomedicine telah digunakan sebagai terapi kanker. Masyarakat indonesia sering mengkombinasikan beberapa tanaman untuk mengobati kanker tanpa data klinis dan data ilmiah yang jelas. Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui metode ekstraksi yang tepat dalam menghasilkan kadar fenol total dan flavonoid total yang paling tinggi, mengetahui efek pemberian ekstrak daun salam, ekstrak daun sirih merah dan kombinasinya terhadap ekspresi dan aktivasi HSP70 dan kadar ROS pada sel HeLa CCL-2. Metode: Ekstraksi dingin (maserasi) dan esktraksi panas (soxhlet) dengan pelarut etanol dibandingkan untuk menghasilkan ekstrak daun salam dan sirih merah. Ekstrak diuji secara kuantitatif dengan uji fenol total dan flavonoid total. Mekanisme antikanker melalui HSP70 ditentukan dengan imunohistokimia dan kadar ROS ditentukan dengan reagen NBT. Hasil: Kadar fenol total paling tinggi diperoleh dengan metode ekstraksi soxhlet sedangkan kadar flavonoid total paling tinggi diperoleh dengan metode ekstraksi maserasi. Ekstrak tunggal dan kombinasinya tidak dapat menurunkan ekspresi HSP70 pada sel HeLa dengan nilai indeks ekpresi HSP70 sama pada semua perlakuan yaitu 1,000. Ekstrak tunggal dapat menurunkan aktivasi HSP70 secara signifikan (p=0,000), tetapi dari hasil uji Tukey hanya salam soxhlet yang dapat menurunkan indeks aktivasi HSP70 menjadi 0,632 secara signifikan (p=0,000). Sedangkan pemberian ekstrak kombinasi tidak berpengaruh terhadap indeks aktivasi HSP70 (p=0,499). Pemberian ekstrak tunggal dapat meningkatkan kadar ROS pada sel HeLa sampai 237,217% secara signifikan (p=0,001) begitu pula dengan pemberian ekstrak kombinasi sampai 244,013% (p=0,001) tetapi tidak ada perbedaan yang signifikan antar dosis yang digunakan. Kesimpulan: Esktrak salam soxhlet memiliki aktivitas antikanker melalui penurunan aktivasi HSP70 dan peningkatan kadar ROS, sedangkan ekstrak salam maserasi, sirih merah maserasi dan ekstrak kombinasi melalui peningkatan kadar ROS pada sel HeLa. Semua ekstrak tidak berpengaruh terhadap ekspresi HSP70.

English Abstract

Background: Eugenia polyantha and Piper crocatum leaves have been used to treat cancer in ethnomedicine. Indonesian people have a habit to combine various plants without unclear scientific and clinical bases. Aims: In this study, we determine extraction methods of the plant leaves to obtain highest total phenolic content and total flavonoid content, examine their potential molecular mechanisms both as single extract and as their combination via expression of HSP70, activation of HSP70 and level of ROS on HeLa cells. Methods: Ethanolic cold (meceration) and hot (soxhlet) extraction methods were compared to extract the leaves. Extracts were evaluated quantitatively for total phenolic content and total flavonoid content. Their molecular mechanisms were evaluated by immunohistochemistry for HSP70 expression and HSP70 activation and NBT reagen for ROS level. Results: The highest total phenolic content was obtained by hot extraction (soxhlet) and highest total flavonoid content was obtained by cold extraction (meceration). This extracts alone and in combination did not have effect in expression index of HSP70 on HeLa cells. Single extract significantly decreased activation of HSP70 (p=0.000), but only soxhlet extract of Eugenia polyantha which significantly decreased activation of HSP70 up to 0.632 by Tukey (p=0.000). Combination extracts did not have effect in activation of HSP70 (p=0.499). Single extract significantly increased ROS level on HeLa cells up to 237.217% (p=0.001) as well as combination extract up to 244.013% (p=0.001) but there was no significant difference of level ROS between doses by Tukey. Conclusions: Soxhlet extract of Eugenia polyantha leaves showed anticancer activity via decreased activation HSP70 and increased ROS level, while macerated Eugenia polyantha, macerated Piper crocatum and combination extracts via increased ROS level only. Single extract and their combination did not have effect in expresssion HSP70.
