Abstract
Latar Belakang: Penyelaman menyebabkan perubahan fungsi paru masih perlu dibuktikan. Stress oksidatif dan decompression sickness akibat penyelaman menyebabkan kerusakan dan inflamasi saluran nafas. Mediator inflamasi yang berperan diantaranya MDA dan LTB4. Tujuan penelitian ini untuk membuktikan pengaruh penyelaman terhadap penurunan fungsi paru, perubahan kadar MDA dan kadar LTB4 serta membuktikan adanya hubungan penurunan fungsi paru, kadar MDA dan LTB4 dalam serum penyelam TNI-AL. Metode: Quasi-eksperimental, 40 sampel terdiri dari 20 penyelam yang berdinas di sekolah penyelam TNI-AL Surabaya dan 20 bukan penyelam TNI-AL yang berdinas di Akademi TNI-AL Surabaya. Kelompok penyelam dan bukan penyelam TNI-AL bulan 1 dilakukan pemeriksaan fungsi paru, pengukuran kadar MDA dan LTB4 dalam serum sebagai data awal. Pada kelompok penyelam sampai dengan bulan 6 dan telah melakukan penyelaman sebanyak 30 kali, dilakukan pemeriksaan kembali fungsi paru, kadar MDA dan LTB4 dalam serum. Pemeriksaan fungsi paru menggunakan alat spirobank II, kadar MDA dan LTB4 menggunakan metode ELISA. Hasil: Penurunan fungsi paru berbeda bermakna FVC (p<0.001), FEV1 (p=0.043), PEF (p<0.001), FEF25 (p<0.001), FEV3 (p<0.001) dan terdapat peningkatan bermakna kadar MDA (p<0.001) dan LTB4 (p=0.003) bulan 1 dan bulan 6. Terdapat hubungan positif sedang bermakna perubahan fungsi paru FEV1 terhadap peningkatan kadar MDA (p=0.044), sedangkan hubungan perubahan fungsi paru dengan peningkatan kadar LTB4 adalah negatif lemah tidak bermakna. Kesimpulan: Penyelaman mengakibatkan perubahan fungsi paru, peningkatan kadar MDA dan LTB4 dalam serum penyelam TNI-AL. Didapatkan hubungan positif sedang bermakna peningkatan fungsi paru terhadap peningkatan kadar MDA dan hubungan negatif lemah tidak bermakna penurunan fungsi paru dengan peningkatan kadar LTB4.
English Abstract
Background: Diving causes lung function changes is still need to be proven. Oxidative stress and decompression sickness caused by diving result in injury and inflammation of airway. Inflammatory mediators such as MDA and LTB4 play role in it. The purposes of this study are to prove the effects of diving toward the decreasing of lung function, to know the changing levels of MDA and LTB4 and to prove the correlation among the decreasing of lung function, MDA and LTB4 levels in the serum of Indonesian Navy divers. Methods: Quasi-experimental, 40 samples consists of 20 divers who served in Indonesian Navy diving course Surabaya and 20 non-divers of Indonesian Navy who served in Academy of Indonesian Navy Surabaya. On the first month, both divers and non-divers had lung function examination and measurement of MDA and LTB4 serum levels as the baseline. Divers group who had dived 30 times (for 6 months) underwent re-examination of lung function, serum levels of MDA and LTB4. Lung function examination using spirobank II, while levels of MDA and LTB4 using ELISA method. Result: There is significantly decreased of lung function; FVC (p<0.001), FEV1 (p=0.043), PEF (p<0.001), FEF25 (p<0.001); significantly increased levels of MDA (p<0.001) and LTB4 (p=0.003) in Indonesian Navy divers group. There are moderate positive significant relationship between lung function changes (FEV1) and increased levels of MDA (p=0.044); and weak negative not significant relationship between lung function changes (FEV1) and increased levels of LTB4 (p=0.223). Conclusion: Diving results in lung function changes, increased levels of MDA and LTB4 in the serum of Indonesian Navy divers. There are moderate positive significant relationship between increased lung function and increased levels of MDA; and weak negative not significant relationship between decreased of lung function and increased levels of LTB4.
