Abstract
Sampai saat ini keberhasilan vaksin BCG dalam memberikan perlindungan terhadap tuberkulosis (TB) pada orang dewasa di Indonesia sebesar 37%, oleh karena itu diperlukan vaksin alternatif yang lebih efektif. Protein rekombinan fusi ESAT6-CFP10 merupakan kandidat vaksin yang potensial. Vaksin subunit merupakan vaksin yang paling banyak diteliti, salah satunya adalah protein rekombinan fusi ESAT6-CFP10. IL2 dan IL10 sel T CD8 memainkan peran penting dalam respon imun melawan TB. Untuk memenuhi tujuan tersebut persentase IL2 dan IL10 sel T CD8 diukur sebagai respon terhadap paparan protein rekombinan fusi ESAT6-CFP10 pada PBMC dari kelompok sehat, kontak TB, dan pasien TB. Setiap kelompok diberi perlakuan tanpa antigen, PPD, dan protein rekombinan fusi ESAT6-CFP10. Setelah dipapar protein rekombinan fusi ESAT6-CFP10, terjadi peningkatan persentase IL2 yang signifikan (p = 0,019) antar kelompok, namun demikian sama yang terjadi pada kelompok tanpa paparan antigen (p = 0,026) antar kelompok. Sebaliknya peningkatan persentase IL2 antar kelompok yang dipapar PPD tidak signifikan secara statistik (p = 0,396). Persentase IL10 tidak menunjukkan perbedaan yang signifikan antar kelompoknya baik tanpa paparan antigen (p = 0,617), PPD (p = 0,351), maupun protein rekombinan fusi ESAT6-CFP10 (p = 0,257). Didapatkan persentase IL2 yang tidak berbeda secara signifikan antar perlakuan pada kelompok sehat (p = 0,309), kelompok kontak TB (p = 0,318), dan kelompok pasien TB (p = 0,424). Demikian juga dengan persentase IL10 yang tidak berbeda secara signifikan antar perlakuan pada kelompok sehat (p = 0,908), kelompok kontak TB (p = 0,352), dan kelompok pasien TB (p = 0,776). Studi ini menunjukkan bahwa protein fusi rekombinan ESAT6-CFP10 dapat meningkatkan persentase IL2 tetapi tidak dengan IL10. Akan tetapi peningkatan persentase IL2 akibat paparan protein rekombinan fusi ESAT6-CFP10 tidak berbeda secara signifikan jika dibandingkan dengan tanpa antigen ataupun dengan paparan PPD.
English Abstract
Until now, the success of the BCG vaccine in providing protection against tuberculosis (TB) in adults in Indonesia is 37%, therefore it urges to find more effective alternative vaccines. ESAT6-CFP10 recombinant fusion protein is a potential vaccine candidate. The subunit vaccine is the most studied vaccine, one of which is the ESAT6-CFP10 recombinant fusion protein. ESAT6 and CFP10 can stimulate IL2 and IL10 CD8 T cells which play an important role against TB. To meet these objectives, the expression of IL2 and IL10 CD8 T cells was measured in response of ESAT6-CFP10 recombinant fusion proteins in the healthy group, TB contacts, and TB patients. Each group was treated without antigen, PPD, and ESAT6-CFP10 recombinant fusion protein. Comparison between groups after exposure to ESAT6-CFP10 recombinant fusion protein, showed a significant difference in the percentage of IL2 (p = 0.019), as well as the percentage of IL2 between groups without antigen exposure (p = 0.026) can be seen a significant increase from healthy group, TB contact, to TB patients. Conversely, an increase in the percentage of IL2 between groups exposed to PPD was not statistically significant (p = 0.396). Unfortunately the percentage of IL10 did not show a significant difference between groups either without antigen exposure (p = 0.617), PPD (p = 0.351), and ESAT6-CFP10 recombinant fusion protein (p = 0.257). The percentage of IL2 was not significantly different between treatments in the healthy group (p = 0.309), contact group TB (p = 0.318), and groups of TB patients (p = 0.424). Likewise with the percentage of IL10 which did not differ significantly between treatments in the healthy group (p = 0.908), the TB contact group (p = 0.352), and the TB patients group (p = 0.776). This shows that the ESAT6-CFP10 recombinant fusion protein can increase the percentage of IL2 but not IL10. However, the increase in the percentage of IL2 due to exposure to ESAT6-CFP10 recombinant fusion protein was not significantly different when compared to without antigen or PPD exposure.
