Abstract
Penyakit infeksi Dengue adalah penyakit yang sering terjadi pada daerah tropis seperti Indonesia. Jumlahnya kian meningkat dari tahun ke tahun. Penyakit ini umumnya dinilai dari kondisi klinis pasien, jumlah trombosit dan nilai hematokrit sebagai indikator terjadinya kebocoran plasma. Jumlah leukosit, limfosit, monosit dan neutrofil seringkali diabaikan walaupun pada infeksi virus biasanya disertai dengan leukopenia, limfositosis, monositosis dan neutropenia. Oleh sebab itu dilakukan analisis mendalam terhadap parameter tersebut sebagai prediktor terjadinya Sindroma Syok Dengue (SSD). Penelitian ini merupakan penelitian cohort retrospektif menggunakan rekam medis subyek anak yang dirawat dari bulan Mei 2016-April 2017 di RS Saiful Anwar Malang. Subjek kasus dipilih secara consecutive. Subjek terdiri dari 50 dengan rincian 23 subjek non syok dan 27 subjek syok. Dengan analisis ROC curve didapatkan subyek DBD dengan leukopenia mempunyai risiko mengalami syok 0,9 kali lebih besar sehingga tidak dapat dijadikan prognosis, limfositosis mempunyai resiko mengalami syok 1,5 kali lebih besar, monositosis mempunyai resiko mengalami syok 1,8 kali lebih besar dan neutropenia mempunyai resiko 1,2 kali lebih besar namun nilai diagnostiknya rendah sehingga tidak dapat dijadikan prognosis. Dari penelitian ini dapat disimpulkan bahwa limfosit dan monosit bisa dipakai sebagai prediktor terjadinya syok pada anak dengan Demam Berdarah Dengue (DBD).
English Abstract
Dengue infection is a common disease in tropical regions such as Indonesia. The number is increasing every year. The disease is generally assessed from the patient's clinical condition, platelet count and hematocrit value as an indicator of plasma leakage. The number of leukocytes, lymphocytes, monocytes and neutrophils is often overlooked even in viral infections usually accompanied by leukopenia, lymphocytosis, monocytosis and neutropenia. Therefore, an in-depth analysis of these parameters was used as a predictor of the occurrence of Dengue Shock Syndrome (DSS). This study is a retrospective cohort study using a child's medical record of children treated from May 2016-April 2017 at Saiful Anwar Hospital Malang. The case subject is chosen consecutively. Subjects consisted of 50 with details of 23 non-shock subjects and 27 subjects of shock. With ROC curve analysis, the subject of dengue fever with leukopenia had a risk of having 0.9 times greater shock so that it could not be used as prognosis, lymphocytosis had a risk of shock 1.5 times greater, monocytosis had a risk of shock 1.8 times greater and neutropenia has a risk of 1.2 times greater but low diagnostic value so can not be used as a prognosis. From this research can be concluded that lymphocytes and monocytes can be used as a predictor of shock in children with Dengue Hemorrhagic Fever.
