Pengaruh Pemberian Vaksin Outer Membrane Protein Porphyromonas Gingivalis Dengan Ajuvan Alum Terhadap Penebalan Miokardium Tikus Model Hipertensi

Abstract

Hipertensi merupakan salah satu penyakit yang dapat menimbulkan kerusakan organ lainnya. Hipertensi yang kronis dan persisten akan menimbulkan komplikasi berupa penebalan miokardium pada Left Ventricular Hyperthropy (LVH). Penebalan miokardium dapat disebabkan oleh peningkatan Systemic Vascular Resistance (SVR) karena disfungsi endotel akibat Reactive Oxygen Species (ROS). Peningkatan ROS menghasilkan peroksidase lipid berupa Malondialdehyde-Modified Low-Density Lipoprotein (MDA-LDL). Rgp atau Arginin Spesific Gingipain pada Outer Membrane Protein (OMP) P.gingivalis terbukti memiliki mimikri terhadap MDA-LDL sehingga membentuk antibodi terhadap MDA-LDL yang akan menghambat disfungsi endotel sehingga akan menurunkan SVR. Penelitian ini bertujuan menganalisis pengaruh pemberian vaksin OMP P.gingivalis dengan ajuvan Alum terhadap penebalan miokardium dengan menggunakan true experimental design secara in vivo di laboratorium dengan metode Randomized Post Test Only Controlled Group Design. Pada awal penelitian, ketiga kelompok perlakuan diinjeksi vaksin OMP P.gingivalis secara intraperitoneal. 2 minggu berikutnya diberikan induksi hipertensi menggunakan injeksi Deoxycorticosterone Acetat (DOCA) 2x/minggu selama 6 minggu secara subkutan dan minum NaCL 1%. Vaksinasi dilakukan pengulangan sebanyak 3 kali dengan rentang waktu 10 hari. Tikus Wistar usia 6-8 minggu dikelompokkan menjadi 5 kelompok yaitu control negatif, control positif (diinjeksi DOCA 10 mg/kgBB), Perlakuan 1 (DOCA 10 mg/kgBB+vaksin OMP P. gingivalis 200ng/injeksi); Perlakuan 2 (DOCA 10 mg/kgBB+vaksin OMP P. gingivalis 200 ng/injeksi+ajuvan Alum 0,1 ml/injeksi); Perlakuan 3 (DOCA 10 mg/kgBB+ajuvan Alum 0,1 ml/injeksi). Kemudian dilakukan pengukuran tekanan darah dan pembedahan untuk mengambil organ ventrikel kiri jantung dan dilakukan pengecatan Hematoxyllin Eosin (HE). Uji Oneway ANOVA menunjukkan nilai signifikan sebesar 0,540 (p>0,05) sehingga dapat disimpulkan pemberian vaksin OMP P. gingivalis dengan ajuvan Alum tidak berpengaruh terhadap penebalan miokardium ventrikel kiri jantung tikus yang diinduksi garam DOCA.

English Abstract

Hypertension is one disease that can cause damage to other organs. Chronic and persistent hypertension will lead to myocardial thickening in Left Ventricular Hyperthropy (LVH). Myocardial thickening may be caused by increased Systemic Vascular Resistance (SVR) due to endothelial dysfunction due to Reactive Oxygen Species (ROS). Increased ROS caused lipid peroxidase that produced Malondialdehyde-Modified Low-Density Lipoprotein (MDA-LDL). Rgp or Arginin Spesific Gingipain in Outer Membrane Protein (OMP) P.gingivalis proved to have mimicry against MDA-LDL to form antibodies against MDA-LDL will inhibit endothelial dysfunction in order to decrease SVR. This study aims to analyze the effect of OMP P.gingivalis vaccine with adjuvant Alum on myocardial thickening by using true experimental design in vivo in laboratory with Randomized Post Test Only Controlled Group Design method. At the beginning of the study, three treatment groups were injected OMP P.gingivalis vaccine intraperitoneally. After 2 weeks, Deoxycorticosterone Acetat (DOCA) was given subcutaneously twice a week for 6 weeks and drinking 1% NaCL. Vaccination was repeated 3 times in every 10 days. Wistar rats aged 6-8 weeks were divided into 5 groups: negative control, positive control (injected DOCA 10 mg / kgBW), Treatment 1 (DOCA 10 mg / kgBB + vaccine OMP P. gingivalis 200ng / injection); Treatment 2 (DOCA 10 mg / kgBB + vaccine OMP P. gingivalis 200 ng / injection + adjuvant Alum 0.1 ml / injection); Treatment 3 (DOCA 10 mg / kgBB + adjuvant Alum 0.1 ml / injection). Then the measurement of blood pressure and surgery conducted to take left ventricular organs of the heart and performed Hematoxyllin Eosin (HE). The results of the analysis with the Oneway ANOVA test showed a value of significance 0.540 (p> 0.05) so it can be concluded that the administration of OMP P. gingivalis vaccine with adjuvant Alum had no effect on the thickening of the left ventricular heart myocardium of rat induced DOCA salt.
