Abstract
Bedah sesar merupakan tindakan persalinan buatan dimana janin dilahirkan melalui pembedahan pada dinding abdomen dan dinding uterus. Tingginya persalinan sesar mendorong ketepatan pemilihan antibiotik untuk mencegah infeksi pascaoperasi. Penelitian bertujuan untuk melakukan analisis cost-effectiveness (CEA) terhadap antibiotik profilaksis seftriakson dan sefotaksim dilihat dari perspektif penyedia layanan kesehatan. Pengumpulan data dilakukan secara retrospektif menggunakan data rekam medis dan keuangan pasien. Jumlah sampel yang diperoleh sebanyak 89 pasien. Analisis data dilakukan dengan menghitung Average Cost-Effectiveness Ratio (ACER) dan Incremental Cost-Effectivemess (ICER). Berdasarkan perhitungan ACER, didapatkan bahwa antibiotik profilaksis yang lebih cost-effective dari segi lama rawat inap dan lama kering luka operasi adalah pasien yang menggunakan seftriakson generik dibandingkan seftriakson paten; pasien dengan seftriakson generik dibandingkan sefotaksim paten; pasien dengan sefotaksim generik dibandingkan seftriakson paten, dan pasien yang menggunakan sefotaksim generik dibandingkan sefotaksim paten. Berdasarkan ACER dan ICER, pasien yang menggunakan sefotaksim paten lebih cost-effective dari segi lama kering luka operasi, sedangkan dari segi lama rawat inap pasien dengan seftriakson paten memerlukan biaya tambahan Rp 152.194 untuk menghasilkan pengurangan 1 hari lama rawat inap. Pada pasien yang menggunakan seftriakson generik memerlukan biaya tambahan Rp 3.916 untuk menghasilkan pengurangan 1 hari lama kering luka operasi serta memerlukan Rp 14.043 untuk menghasilkan pengurangan 1 hari lama rawat inap. Sehingga dapat disimpulkan bahwa antibiotik profilaksis yang lebih cost-effective adalah sefotaksim generik.
English Abstract
Caesarean section is a method of labor when the fetus is born through the abdominal wall and on the uterine. High prevalence of C-section needs rational use of prophylaxis antibiotic important to decrease the risk of infection that comes post-operative. The aim of this study was to analyze the use of intravenous ceftriaxone and cefotaxime by using CEA method in C-section patients from the perspective of health care providers. The study was conducted retrospectively from medical record and direct medical cost of patients. Total samples obtained were 89 patients. Pharmacoeconomic analysis method of this research was CEA with the calculation of the Average Cost-Effectiveness Ration (ACER) and Incremental Cost-Effectiveness Ratio (ICER). Based on the calculation of ACER, antibiotic therapy which was more cost effective based on duration of hospitalization and duration of wound to dry was generic ceftriaxone than patent ceftriaxone; patients using generic ceftriaxone than patent cefotaxime; patients using generic cefotaxime than patent ceftriaxone; and patients using generic cefotaxime than patent cefotaxime. Based on the calculation of ACER and ICER, the payment of patent cefotaxime was more cost-effective in terms of duration of wound to dry, whereas in terms of long hospitalization in patients using patent ceftriaxone required Rp 152.194 per day as an additional cost. For patients using generic ceftriaxone required Rp 3.916 per day as additional cost for reduction in duration of wound to dry and required Rp 14.043 per day for reduction in hospitalization. It can be concluded that the more cost-effective antibiotics is generic cefotaxime.
