Abstract

Program sistem sms gateway puskesmas Cengkareng (Si Gareng) merupakan sebuah inovasi pelayanan kesehatan dengan tujuan meningkatkan kepatuhan berobat pada penderita diabetes mellitus (DM) melalui pelayanan pendaftaran, konsultasi, serta reminder berbasis sms. Dalam jangka pendek program ini diharapkan dapat meningkatkan kepatuhan berobat dan kesadaran pasien terhadap penyakit DM. Sedangkan dalam jangka panjang program Si Gareng dapat menjadi contoh inovasi pelayanan publik khususnya di bidang kesehatan pada daerah lain. Penelitian ini bertujuan untuk menganalisis proses inovasi program Si Gareng dan untuk menganalisis faktor pendukung beserta penghambat proses inovasi program Si Gareng. Penelitian ini menggunakan jenis penelitian kualitatif dengan pendekatan studi kasus tunggal yang berlokasi di Kecamatan Cengkareng, sedangkan situsnya berada di Puskesmas Kecamatan Cengkareng. Wawancara dilakukan kepada Kepala Puskesmas Kecamatan Cengkareng, pencetus program, pasien sekaligus pengguna program Si Gareng, serta informan pendukung lainnya. Teknik analisis data menggunakan Yin (2009) dalam menganalisis kasus proses inovasi program Si Gareng serta faktor pendukung dan penghambatnya. Hasil penelitian ini menunjukkan program Si Gareng telah berjalan sejak Januari 2015 hingga saat ini dan telah berhasil meningkatkan persentase kepatuhan berobat pasien DM puskesmas kecamatan Cengkareng dari 30,5% menjadi 75% serta meningkatkan kesadaran pasien DM dalam kontrol rutin. Proses inovasi program Si Gareng dimulai dari identifikasi masalah yang dilakukan oleh seorang dokter di Poli PTM puskesmas kecamatan Cengkareng, pemunculan ide sebagai solusi permasalahan rendahnya kepatuhan berobat pada penyakit DM, pengembangan proposal program Si Gareng, penerapan inovasi program Si Gareng di Poli PTM, evaluasi inovasi program Si Gareng, dan yang terakhir penyebaran inovasi program kepada instansi maupun daerah lain. Faktor pendukung proses inovasi program Si Gareng terdiri dari adanya dukungan Kepala Puskesmas, keselarasan tim Gendhis, kemitraan dengan pihak TI swasta, penghargaan atas kinerja inovasi. Sedangkan faktor penghambat proses inovasi program Si Gareng, yaitu budaya organisasi puskesmas yang kurang menerima inovasi, kurangnya pelatihan TI pegawai puskesmas, serta berbelit-belitnya birokrasi dalam penyebaran inovasi dan pemberian anggaran. Saran yang ditemukan pada penelitian ini, yaitu merubah cara pandang dan kesadaran pasien, penanaman budaya inovatif, pengadaan pelatihan bulanan pegawai TI, pengembangan program melalui media sosial.

English Abstract

Short Massage Service (SMS) gateway system of Cengkareng health center (Si Gareng) program is an innovative health service witch have purpose to improving treatment adherence for diabetes mellitus (DM) patients through sms registration, consultation and reminder services. In short term this program is expected to improve treatment compliance and patient behavior against DM disease. While in long term, Si Gareng program can be expected to become an example of public service innovation, especially the health sector in other areas. This study purposed to analyze the innovation process of Si Gareng program and to analyze the supporting and inhibiting factors innovation program of Si Gareng. This research used qualitative research with single case study approach which located in Cengkareng district, while the site was in Cengkareng health center. Interviews conducted to leader of Cengkareng Health Center, the originator of Si Gareng program, patient and user of Si Gareng, and other supporting informants. Techniques explanation analysis of Yin (2009) to analyzing case of an innovation process Si Gareng program and their supporting and inhibiting factors. The results of this study shows that Si Gareng program has been running since January 2015 until now and has succeeded in increasing the percentage of treatment compliance of DM patient of Cengkareng health center from 30,5% to 75% and increasing awareness of DM patient in routine control. The process of innovation Si Gareng program started from the identification of problem which has been done by a doctor in Poly PTM of Cengkareng health center, generating the idea as solution of low compliance problem in DM disease, developing proposal Si Gareng Program, implementation this innovation Si Gareng program in Poly PTM, evaluation of innovation Si Gareng program, and the last diffusing lessons of innovation program to other institution or region. The supporting faktors innovation Si Gareng program consist of support from leader of health center, Gendhis team alignment, partnership with IT private sector, and awards for innovation performance. While the inhibiting factors innovation Si Gareng program from the organizational culture of the health center that less received innovation, lack of IT training staff health center, and bureaucratic convoluted in the diffusing and budgeting innovation. The found suggest of this study are include patient perspective and awareness, innovative cultural cultivation, provision of IT workforce training, developing program through social media.
