Abstract

Latar Belakang: Metode kasus pernah diterapkan pada 2011-2015 di IGD RSUD Ngudi Waluyo Wlingi Kabupaten Blitar tapi tidak berhasil. Hasil ini bisa dilihat dari LOS pasien masih ˃ 8 jam.. Januari 2017 metode kasus diaplikasikan kembali yang bertujuan untuk menurunkan LOS. Namun samapai saat ini belum pernah dievaluasi. Tujuan:Mengevaluasi Penerapan Metode Kasus dan Pengaruhnya terhadap Length of Stay di IGD RSUD Ngudi Waluyo Wlingi Kabupaten Blitar. Metode: Jenis penelitian ini adalah analitik observasional dengan pendekatan cross sectional. Sampling yang digunakan adalah total sampling dengan total sampel berjumlah 23 perawat dan kuota sampling dengan total sampel 100 orang pasien. Variabel Independen terdiri dari metode kasus dan variable dependen LOS. Analisis yang digunakan adalah korelasi spearman, regresi logistik sederhana dan regresi logistik berganda. Hasil:Hasil penelitian menunjukkan bahwa LOS ≤ 6 jam sebanyak 62 % dan ˃ 6 jam sebanyak 38 %. Berdasarkan hasil analisis bivariat variabel yang signifikan yaitu metode kasus (p value = 0.032), laboratorium (p value = 0.024), konsul (p value = 0.015), dan jumlah sift jaga (p value = 0.035). Berdasarkan hasil uji multivariat dengan regresi logistic berganda disimpulkan bahwa faktor yang paling berpengaruh terhadap LOS adalah kecepatan konsultasi dokter yaitu p value 0,013 dan nilai OR 0,901 Kesimpulan & Saran: Kecepatan konsultasi dokter adalah yang paling dominan berpengaruh dengan LOS di IGD RSUD Ngudi Waluyo Wlingi Kabupaten Blitar, karena itu disarankan penambahan dokter spesialis dengan sup spesialis yang jaga di IGD RSUD Ngudi Waluyo Wlingi Kabupaten Blitar.

English Abstract

Background: The case method was applied in 2011 until 2015 in Emergency Unit of Ngudi Waluyo Public Hospital Blitar District but it was successful. This result can be seen from patient,s length of stay that more than 8 hours. Starting from january 2017the case method was applied again.However, it has not yet been evaluated. Objective:To evaluate LOS the the application of case method and its influence on the LOS in Emergency Unit Ngudi Waluyo Public Hospital Wlingi District of Blitar. Method: Research design was observational analytic with cross sectional approach. The sampling used total sampling with total samples of 23 nurses and quota sampling with total samples of 100 patients. As independent variablel are the case method and depedent variable which are length of stay. The analysis that used speraman corelations, logistik bivariate simple and logistik multivariate multiple. Result:The results showed that LOS ≤ 6 hours as much as 62% and ˃ 6 hours as much as 38%. Based on the results of bivariate its significant variable of the case method (p 0,032),laboratory examination rate (p = 0.024), physician consultation rate (p = 0.015), number of nurses each shift (p = 0.035). Analysis result of multiple linear regression multivariates of three factors, the most dominant and strong correlation of LOS was physician consultation rate with p value 0.007 and OR (0,102). Conclusion & Recommendation: Physician consultation rate was the most dominant related to emergency installation of Ngudi Waluyo Public Hospital Wlingi District of Blitar because recomended addition doctor specialist with subspecialty in Emergency Unit of Ngudi Waluyo Public Hospital Blitar District.
