Abstract

Latar Belakang: ISK merupakan penyakit infeksi yang sering dijumpai. ISK sering bergejala ringan, walaupun terkadang bisa timbul komplikasi berat yaitu sepsis. Pemeriksaan baku emas untuk menegakkan diagnosis ISK adalah kultur urine, tetapi pemeriksaan ini membutuhkan waktu yang lama, karena itu diperlukan diagnosis dini dari ISK. Tujuan penelitian untuk mencari petanda biologi diagnosis ISK yang lebih cepat dibanding kultur urine. HBP adalah homolog inaktif dari serine protease yang ada di dalam netrofil, yang dilepaskan dari neutrofil pada proses inflamasi. LE adalah suatu enzim yang diproduksi oleh leukosit terutama neutrofil, LE dilepaskan oleh granul dari neutrofil saat lisis setelah memfagosit antigen. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain potong lintang yang dilakukan selama 4 bulan terhadap 52 pasien dengan gejala ISK dan 16 subyek kontrol sehat. Kadar U-HBP diperiksa dengan metode ELISA sedangkan LE diperiksa menggunakan carik celup. Hasil: Nilai diagnostik untuk U-HBP didapatkan sensitivitas 96,08%, spesifisitas 94,12%, Nilai Ramal Positif (NRP) 98,00%, Nilai Ramal Negatif (NRN) 88,89% dan untuk LE didapatkan nilai sensitivitas 76,47%, spesifisitas 94,12%, NRP 97,50%, NRN 57,14%. Didapatkan korelasi yang positif lemah antara kadar U-HBP dan LE pada penderita ISK dengan r: 0.378 (p: 0,000) Kesimpulan: U-HBP memiliki nilai diagnostik yang lebih bagus daripada LE untuk diagnosis ISK

English Abstract

Background: UTI is a commonly encountered infection disease. Although UTI is often asymptomatic or has mild symptoms, sometimes a severe complication such as sepsis might occur. Gold Standard examination for UTI is urine culture, but this examination requires a lengthy time, therefore another examination with shorter time is needed. The purpose of this study is to find an early diagnostic marker of UTI. HBP is an inactive homolog of serine protease that resides within neutrophil and released from neutrophil during inflammation. LE in an enzyme produced by leukocyte mainly neutrophil, LE is released by neutrophil's granules when neutrophyl is lysed after fagotizing antigen. Methods: This is a cross sectional observational analytic study performed for 4 months towards 52 pastient with UTI symptoms and 16 healthy controls. U-HBP are examined using ELISA while LE are examined using dipstick.. Results: U-HBP has a sensitivity of 96.08%, spesificity of 94.12%, Positive predictive value (PPV) of 98.00%, negative predictive value (NPV) of 88.89% while LE has 76.47% sensitivity, 94.12% specificity, 94.12% PPV and 97.50% NPV. A weak positive correlation is found between U-HBP and LE in UTI patients with r: 0.378. Conclusions: U-HBP has a better diagnostic value in diagnosing UTI when compared to LE..
