Abstract
Prevalensi pasien kritis yang mendapatkan perawatan khusus di ruang ICU (Intensive Care Unit) meningkat setiap tahunnya. Pasien sakit kritis mengalami respon metabolik dan respon inflamasi. Salah satu penanda terjadinya inflamasi adalah tingginya rasio trombosit limfosit (Platelet lymphocyte Ratio /PLR). Respon inflamasi akan menyebabkan malnutrisi apabila tidak diatasi dengan benar. Pemberian enteral nutrition yang tepat diperlukan untuk mencegah pasien jatuh ke dalam kondisi yang lebih parah dan berkepanjangan. Penelitian ini bertujuan untuk mengetahui hubungan pemberian enteral nutrition terhadap rasio trombosit limfosit (PLR) pada pasien ICU RSUD Dr. Saiful Anwar Provinsi Jawa Timur. Penelitian ini merupakan penelitian analitik observasional dengan pendekatan cross-sectional. Pemilihan sampel menggunakan teknik purposive sampling yang berjumlah 15 subjek penelitian. Variabel yang diukur adalah asupan energi dan zat gizi selama 3 hari pemberian enteral nutrition dan PLR subjek penelitian. Data asupan enteral nutrition dan PLR didapatkan dari data sekunder berupa kardek dan data rekam medis. Uji hubungan menggunakan Pearson. Hasil penelitian menunjukkan bahwa rata-rata asupan energi penelitian 1054,05±398,27 kkal, rata-rata asupan karbohidrat 153,58±59,37 g, rata-rata asupan protein 41,42±19,34 g, rata-rata asupan lemak 28,98±11,91 g, rata-rata asupan omega-3 1,40±0,73 g, dan rata-rata PLR 201,07±75,95. Hasil uji Pearson menunjukkan bahwa tidak ada hubungan antara asupan energi, karbohidrat, protein, lemak, dan omega-3 terhadap PLR dengan p-value masing-masing 0,36; 0,29; 0,33; 0,44; 0,67 (p>0,05). Kesimpulan pada penelitian ini adalah pemberian enteral nutririton tidak mempengaruhi rasio trombosir limfosit pasien ICU.
English Abstract
The prevalence of critically ill patients receiving specialized care in the Intensive Care Unit (ICU) is increasing every year. Critically ill patients experience metabolic responses and inflammatory responses. One of the markers of inflammation is the high platelet lymphocyte ratio (PLR). The inflammatory response will cause malnutrition if not properly addressed. Proper enteral nutrition is needed to prevent patients from falling into a more severe and prolonged condition. This study aims to determine the relationship of enteral nutrition to lymphocyte platelet ratio (PLR) in ICU patients at Dr. Saiful Anwar Hospital, East Java Province. This study is an observational analytic study with a cross-sectional approach. Sample selection using purposive sampling technique which amounted to 15 research subjects. The variables measured were energy and nutrient intake during 3 days of enteral nutrition and PLR of the research subjects. Data on enteral nutrition intake and PLR were obtained from secondary data in the form of card and medical record data. Pearson correlation test was used. The results showed that the average energy intake of the study was 1054.05±398.27 kcal, average carbohydrate intake was 153.58±59.37 g, average protein intake was 41.42±19.34 g, average fat intake was 28.98±11.91 g, average omega-3 intake was 1.40±0.73 g, and average PLR was 201.07±75.95. Pearson test results showed that there was no relationship between energy, carbohydrate, protein, fat, and omega-3 intake on PLR with p-values of 0.36; 0.29; 0.33; 0.44; 0.67 respectively (p>0.05). The conclusion of this study is that the administration of enteral nutrition does not affect the lymphocyte thrombocyte ratio of ICU patients.
