Abstract
LATAR BELAKANG: Di Indonesia, prevalensi hipertensi paru adalahh 1 : 10.000 penduduk. Karena gejala yang tidak khas, sering kali pasien dengan hipertensi paru datang dengan gejala lanjut yang mengakhibatkan prognosis yang buruk. Standar baku emas penegakan diagnosis hipertensi paru adalah dengan kateterisasi jantung kanan (RHC) yang saat ini belum banyak tersedia di fasilitas kesehatan di Indonesia. OBJEKTIF: Menciptakan sistem penilaian berdasarkan presentasi klinis, elektrokardiogram, dan rontgen dada untuk deteksi dini. METODE: Survei cross-sectional berbasis rumah sakit dilakukan di RSUD dr. Saiful Anwar, Jawa Timur pada Juli 2020 – November 2023. Data dianalisis menggunakan analisis bivariat, regresi logistik berganda, dan Area Under Curve (AUC) dari Receiver Operating Characteristic (ROC). HASIL: Sebanyak 292 pasien yang dilakukan RHC di RSUD dr. Saiful Anwar Jawa Timur terdaftar dalam penelitian ini tetapi hanya 249 pasien yang memenuhi kriteria inklusi dan eksklusi. Temuan kami menunjukkan variabel yang signifikan adalah Dispnea saat aktivitas (DOE), Palpitasi, P2 mengeras, Peningkatan JVP, Heaves, RBBB, dan CTR >50%. Dengan tabel 2x2, kami menemukan bahwa skor ⩾5 kemungkinan besar untuk diagnosis hipertensi pulmonal. KESIMPULAN: Penelitian kami menunjukkan bahwa sistem penilaian yang kami hasilkan memiliki sensitivitas dan spesifisitas yang baik pada populasi tertentu dimana salah satu keterbatasannya adalah bias pengambilan sampel sehingga belum dapat diterapkan pada populasi umum dan memerlukan penelitian lebih lanjut untuk validasi.
English Abstract
BACKGROUND: In Indonesia, pulmonary hypertension prevalence is 1:10.000 population. Due to its atypical symptoms, often PH patients come as a late presenter thus they have a worse prognosis. The golden standard to diagnose is Right Heart Catheterization (RHC) which is not widely available. OBJECTIVES: To generate a scoring system based on clinical presentation, electrocardiogram, and chest x-ray for early detection. METHODS: A hospital-based cross-sectional survey was conducted in RSUD dr. Saiful Anwar, East Java from July 2020 – November 2023. Data were analyzed using bivariate analysis, multiple logistic regression, and Area Under Curve (AUC) from Receiver Operating Characteristic (ROC). RESULTS: A total of 292 patients who were conducted RHC at RSUD dr. Saiful Anwar East Java was enrolled for the study but only 244 patients were suitable with the inclusion criteria. Our findings showed significant variables were Dyspnea on effort (DOE), Palpitation, Increased P2, Increased JVP, Heaves, RBBB, and CTR >50%. With a 2x2 table, we found that a score of ⩾5 is likely for pulmonary hypertension diagnosis. CONCLUSION: Our study reveals that the scoring system that we generate has good sensitivity and specificity in selected populations where one of its limitations is sampling bias thus cannot yet be applied to the general population and needs further research for validation.
