Abstract

Saat ini non-alcoholic fatty liver disease (NAFLD) menjadi masalah kesehatan dunia yang serius karena tingginya prevalensi obesitas dan diabetes mellitus tipe 2 yang merupakan faktor risiko terbesar NAFLD. Tingginya prevalensi NAFLD berkaitan erat dengan munculnya berbagai komplikasi yang akan berujung pada tingginya permintaan transplantasi liver hingga menyebabkan kematian. Dalam patogenesisnya, jaringan adiposa visceral disebutkan turut berdampak terhadap perkembangan penyakit NAFLD. Berdasarkan banyak penelitian, dikatakan Lactobacillus plantarum Dad-13 dan metformin berpotensi menjadi terapi pasien NAFLD karena banyaknya manfaat yang telah dibuktikan. Tujuan penelitian ini adalah untuk mengetahui pengaruh pemberian kombinasi L. plantarum Dad-13 dan metformin terhadap jaringan adiposa visceral tikus model NAFLD. Subjek yang digunakan dalam penelitian ini adalah tikus Rattus norvegicus galur Wistar berjenis kelamin jantan. Jumlah dan ukuran sel adiposa kemudian dilakukan analisis uji normalitas, uji homogenitas, uji Kruskal-Wallis, dan selanjutnya dilakukan uji beda (post-hoc) Mann-Whitney untuk mengetahui pasangan kelompok mana yang terpengaruh secara signifikan terhadap intervensi. Hasil penelitian ini menunjukkan terdapat pengaruh yang signifikan terhadap penurunan ukuran sel tikus model NAFLD yang diberikan kombinasi L. plantarum dan metformin (p = 0,048) dibandingkan dengan ukuran sel kelompok tikus model NAFLD tanpa intervensi. Selain itu, terdapat peningkatan jumlah sel pada tikus model NAFLD yang diberikan kombinasi L. plantarum dan metformin dibanding kelompok tikus NAFLD tanpa intervensi meskipun secara statistik tidak signifikan (p = 0,655). Kesimpulan dari penelitian ini adalah terdapat perbedaan gambaran histopatologi jaringan adiposa visceral terhadap pemberian kombinasi L. plantarum Dad-13 dan metformin pada tikus model NAFLD sehingga berpotensi menjadi pilihan pengobatan bagi pasien NAFLD. Penelitian lebih lanjut perlu dipertimbangkan dengan menambah kelompok variabel dan durasi pemberian intervensi.

English Abstract

Currently, non-alcoholic fatty liver disease (NAFLD) is a serious world health problem due to the high prevalence of obesity and type 2 diabetes mellitus, both of which are the biggest risk factors for NAFLD. The high prevalence of NAFLD is closely related to various complications, which will lead to high demands for liver transplantation and can even cause death. In its pathogenesis, visceral adipose tissue is said to have impacts on the development of NAFLD. Based on many studies, Lactobacillus plantarum Dad-13 and metformin known to have potential to be therapy for NAFLD patients because of the many benefits that have been proven. This study aimed to determine the effects of giving a combination of Lactobacillus plantarum Dad-13 and metformin on the visceral adipose tissue of NAFLD rat models. The subjects used in this study were male Rattus norvegicus. The number and size of adipose cells were analyzed for normality test, homogeneity test, Kruskal-Wallis test, and then the Mann-Whitney post-hoc test was carried out to find out which group pairs were significantly affected by the intervention. The results of this study showed that there was a significant effect on decrease in cell size of NAFLD rat models given the administration of L. plantarum and metformin combination (p = 0.048) compared to the cell size of the NAFLD rat models group without intervention. Beside that, there was an increase in the number of cells in the NAFLD rat models given the combination of L. plantarum and metformin compared to the NAFLD rat models group without intervention, although not statistically significant (p = 0.655). This study concluded that there are differences in the histopathological features of visceral adipose tissue from the administration of the L. plantarum Dad-13 and metformin combination in NAFLD rat models, so it has potential to be a treatment option for NAFLD patients. Further research by adding the variable groups and the duration of the intervention needs to be considered.
