Abstract

Pneumonia ialah infeksi akut yang penyerangannya pada bagian paru-paru. Pneumonia balita ditandai dengan adanya gejala batuk dengan kesukaran bernapas, seperti napas cepat, tarikan dinding dada bagian bawah ke dalam, atau kadar oksigen dalam darah menurun dengan kadar saturasi oksigen yang rendah. Saturasi oksigen merupakan salah satu indikator kecukupan pasokan oksigen, ketika saturasi rendah menyebabkan pasokan oksigen ke jaringan berkurang. Salah satu intervensi keperawatan non farmakologis dalam meningkatkan saturasi oksigen adalah tindakan nesting. Nesting ialah salah satu upaya yang dilaksanakan dengan memberikan posisi pada seorang bayi di tempat tidur yang telah dimodifikasikan dan dibuat dari kain yang digulung dengan bentuknya menyerupai rahim ibu ketika bayi masih di dalam kandungan. Dari studi awal yang dilakukan pada tanggal 07 April 2022 di R. NICU RST Soepraoen didapatkan data bayi usia 28 hari, laki-laki, lahir prematur usia gestasi 30 minggu, dengan BBLR 2125 gr dan PB 45 cm, kondisi pasien lemah, sesak nafas, batuk, agak demam, kesadaran compos mentis, suhu 37,30C, nadi 182 x/menit, RR: 52 x/menit, SPO2 93%, terpasang CPAP dengan FIO 60%, Flow 6 Lpm, PEEP 7 cmH2O, serta didiagnosis dokter pasien menderita pneumonia. Tujuan penelitian ini untuk mengetahui dampak langsung pengaruh penggunaan nesting terhadap saturasi oksigen pada bayi dengan pneumonia. Pasien telah diberikan asuhan keperawatan selama 3 hari mulai tanggal 07 – 09 April 2022 dengan masalah keperawatan yang muncul pada pasien diantaranya yakni gangguan pertukaran gas dibuktikan dengan hasil pemeriksaan BGA, pH: 7,295, PCO2: 47,8 mmHg (asidosis respiratorik). Perfusi jaringan tidak efektif dibuktikan dengan CRT > 2 detik, akral teraba dingin, edema (+), Hb: 10,8 g/dl, Hematokrit : 29%, pucat. Nyeri akut dibuktikan dengan bayi rewel dan menangis, bersikap protektif, takikardia. Risiko infeksi dibuktikan dengan Leukosit: 662.000 ribu/mm3, Terpasang O2 CPAP. Berdasarkan nilai saturasi oksigen bayi yang diberikan intervensi penggunaan bantal nesting mengalami peningkatan nilai saturasi oksigen dari 93% sampai dengan 99% selama 3 hari perawatan.

English Abstract

Pneumonia is an acute infection that attacks the lungs. Toddler pneumonia is characterized by coughing symptoms with difficulty breathing, such as rapid breathing, pulling the lower chest wall in, or decreasing blood oxygen levels with low oxygen saturation levels. Oxygen saturation is one indicator of the adequacy of oxygen supply, when low saturation causes the oxygen supply to the tissues to decrease. One of the non-pharmacological nursing interventions in increasing oxygen saturation is nesting. Nesting is one of the efforts carried out by placing a baby in a bed that has been modified and made of rolled cloth with a shape resembling the mother's womb when the baby is still in the womb. From an initial study conducted on April 7, 2022 at the R. NICU of RST Soepraoen, data obtained were 28 days old, male, born prematurely at 30 weeks' gestation, with LBW 2125 g and PB 45 cm, the patient's condition was weak, short of breath, cough, slight fever, compos mentis consciousness, temperature 37.30C, pulse 182 x/minute, RR: 52 x/minute, SPO2 93%, CPAP installed with 60% FIO, Flow 6 Lpm, PEEP 7 cmH2O, and diagnosed by the patient's doctor suffering from pneumonia. The purpose of this study was to determine the direct impact of the use of nesting on oxygen saturation in infants with pneumonia. The patient has been given nursing care for 3 days starting on 07 – 09 April 2022 with nursing problems that arise in the patient including gas exchange disorders as evidenced by the results of the BGA examination, pH: 7.295, PCO2: 47.8 mmHg (respiratory acidosis). Ineffective tissue perfusion as evidenced by CRT > 2 seconds, cold akral, edema (+), Hb: 10.8 g/dl, Hematocrit: 29%, pale. Acute pain is evidenced by the baby fussing and crying, being protective, tachycardia. The risk of infection is evidenced by Leukocytes: 662,000 thousand/mm3, O2 CPAP installed. Based on the oxygen saturation value of infants who were given the intervention using a nesting pillow, the oxygen saturation value increased from 93% to 99% for 3 days of treatment.
