Abstract

Artritis reumatoid (AR) merupakan salah satu penyakit inflamasi kronis yang paling sering dijumpai, terutama pada sendi. Durasi sakit merupakan faktor prediktif penting dari kerusakan sendi yang dapat mengakibatkan kerusakan struktural berat jika tidak diobati dalam jangka panjang. Walaupun AR tidak dapat disembuhkan, namun pengobatan sedini mungkin dan pendekatan terapi modern dapat mengontrol progresivitas penyakit dengan sangat baik. Radiografi konvensional menjadi modalitas penting untuk menilai progresivitas penyakit AR dengan menggunakan sistem skoring. Tujuan penelitian ini untuk menganalisis hubungan durasi sakit dan pengobatan AR dengan SHS pada radiografi konvensional manus dan pedis, serta menganalisis kesesuaian SHS dengan skor 3 lokasi sendi yang paling dominan mengalami kerusakan. Penelitian ini merupakan penelitian observasional analitik dengan pendekatan cross sectional, menggunakan data 24 pasien yang telah melaksanakan pemeriksaan radiografi konvensional manus dan pedis dalam satu waktu. Tidak ada hubungan durasi sakit AR dengan SHS, skor erosi tulang, maupun skor penyempitan celah sendi. Namun didapatkan adanya kecenderungan erosi manus dan pedis serta penyempitan celah sendi pedis pada durasi sakit lebih dari 2 tahun. Terdapat hubungan pengobatan AR dengan SHS pada durasi sakit lebih dari 2 tahun antara MTX + KS dan non MTX (p = 0,024), dengan rerata skor MTX + KS paling rendah. Terdapat ketidaksesuaian SHS dengan skor 3 lokasi sendi yang paling dominan mengalami kerusakan pada radiografi konvensional manus dan pedis.

English Abstract

Rheumatoid arthritis (RA) is one of the most common chronic inflammatory diseases, especially in the joints. Disease duration is an important predictive factor of joint damage which can cause severe structural damage if left untreated in the long term. Although RA can not be cured, early treatment dan modern therapeutic approaches can control disease progression very well. Conventional radiography is an important modality to assess the progression of RA by using a scoring system. The purpose of this study was to analyze the relationship of disease duration and treatment of RA with SHS on conventional radiographs of the hands and feet, and to analyze the compatibility of SHS with a score of 3 joint locations with the most dominant damage. This study was an analytic observational study with a cross-sectional design, using data from 24 patients who had performed conventional radiographs examination of the hands and feet at one time. There was no relationship between disease duration of RA and SHS, bone erosion score, or joint space narrowing score. However, it was found that there was a tendency for erosion of the hands and feet and narrowing of the feet joint space in patient with disease duration more than 2 years. There was a relationship between treatment of RA and SHS in patient with disease duration more than 2 years between MTX + KS and non MTX (p = 0,024), with the lowest mean score is MTX + KS. There is a discrepancy in SHS with a score of 3 joints locations with the most dominant damage on conventional radiographs of the hands and feet.
