Abstract
Latar belakang: Pneumonia yang disebabkan oleh infeksi SARS-CoV-2 menginduksi overaktivasi respon imun pada paru dan meningkatkan produksi stress oksidatif. Malondialdehid (MDA) adalah biomarker stres oksidatif. RALE score digunakan untuk menilai kerusakan paru difus pada ARDS dari foto rontgen toraks. N-acetylcysteine (NAC) berperan dalam sintesis glutathione, dapat memerangi stres oksidatif. Penelitian ini bertujuan untuk menganalisis pengaruh terapi adjuvan NAC terhadap kadar MDA dan RALE score sebagai upaya memperbaiki stres oksidatif dan kerusakan paru pada pasien COVID-19. Metode: quasi eksperimen, non-equivalent control group design. Terdapat 91 subjek yang dipilih secara berurutan, 75 subjek diberi NAC dan 16 subjek tanpa NAC, dengan dosis NAC 5000mg/72 jam. Kadar MDA dan RALE score diukur pada hari ke-1 dan ke-8 setelah pemberian NAC. Hasil: Terdapat penurunan MDA yang signifikan pada kelompok D1 sampai D8 pada kelompok NAC (91,38±138,53) dibandingkan kelompok kontrol 859,77±1952,96 (p=0,000). Terdapat penurunan RALE Score yang signifikan pada kelompok NAC 0,8125±11,03 dibandingkan kelompok kontrol -5,91±9,93 (p=0,018). Kesimpulan: Terdapat korelasi positif antara MDA dan RALE score. Terdapat penurunan yang signifikan dari MDA dan RALE Score setelah terapi adjuvant NAC yang menurunkan stres oksidatif dan kerusakan paru pada pasien COVID-19.
English Abstract
Background: Pneumonia caused by SARS-CoV-2 induces overactivation of immune response in the lung and increases production of oxidative stress. Malondialdehyde (MDA) is a biomarker of oxidative stress. The RALE score was used to assess diffuse lung damage in ARDS from a chest X-ray. N-acetylcysteine (NAC) plays a role in the synthesis of glutathione, can combat oxidative stress. This study aimed to analyze the effect of N-Acetylcysteine adjuvant therapy on MDA levels and RALE scores as an effort to reduce oxidative stress and lung damage in COVID-19 patients. Methods: a quasi-experimental method, non-equivalent control group design. There were 91 subjects selected sequentially, 75 subjects were given NAC and 16 subjects were without NAC, with a dose of NAC 5000mg/72 hours. MDA levels and RALE scores were measured on the 1st and 8th days after administration of NAC. Results: There was a significant decrease in MDA in groups D1 to D8 in the NAC group (91.38±138.53) compared to the control group 859.77±1952.96 (p=0.000). There was a significant decrease in the RALE Score in the NAC group 0.8125±11.03 compared to the control group -5.91±9.93 (p=0.018). Conclusion: There is a positive correlation between MDA and RALE scores. There is a a significant decrease in MDA and RALE Score after adjuvant NAC therapy that reduces oxidative stress and lung damage in COVID-19 patients.
