Abstract
Pendahuluan: Artritis reumatoid (AR) merupakan penyakit artritis autoimun yang sering dijumpai, Dalam praktik klinis, penyakit ini memiliki variasi perjalanan klinis yang beragam, baik keparahannya, respon pengobatannya, maupun tingkat kekambuhannya. Patogenesis AR diperankan oleh berbagai macam komponen sistem imun. Prevalensi sel Th1 dalam patogenesis AR masih kontroversial. Penelitian sebelumnya menunjukkan perbedaan dalam sel Th1 dominan dalam sirkulasi pada perjalanan AR. Peningkatan aktivasi Th1 tetap berjalan pada pasien dengan AR lanjut bahkan setelah pengobatan anti-TNF, meskipun ada perbaikan klinis. Sel Treg yang mengekspresikan FOXP3+ dapat dibedakan dari subset sel T lainnya seperti Tr1, Th3 dan CD8+CD28+, yang juga menunjukkan fungsi penekan. Tujuan: membuktikan korelasi antara persentase sel Th1 dan sel Treg dengan derajat aktivitas penyakit dan kerusakan sendi pada pasien AR. Metode: Penelitian ini menggunakan metode deskriptif analitik dengan 28 sampel subjek pasien AR di Poliklinik Reumatologi RSUD Dr Saiful Anwar Malang mulai bulan Mei 2019 sampai dengan Oktober 2022. Hasil: Berdasarkan korelasi spearman terhadap persentase sel Th1, sel Treg, dan rasio sel Th1/Treg dengan aktivitas penyakit (DAS28-LED) berturut-turut menunjukkan hasil nilai p=0,414 (p>0,05), p=0,218 (p>0,05), dan p=0,106 (p>0,05). Dari hasil korelasi spearman terhadap persentase sel Th1, sel Treg, dan rasio sel Th1/Treg dengan kerusakan sendi (Modified Sharp Score) berturut-turut menunjukkan hasil p=0,094 (p>0,05), p=0,246 (p>0,05). Terdapat korelasi negatif antara rasio sel Th1/Treg dengan kerusakan sendi, dengan nilai r=-0,470 and p=0,027 (p<0,05). Kesimpulan: Tidak terdapat korelasi nilai Th1, Treg maupun rasio Th1/Treg dengan derajat keparahan penyakit (DAS28-LED). Terdapat korelasi negatif antara rasio sel Th1/Treg terhadap kerusakan sendi (Modified Sharp Score).
English Abstract
Background: Rheumatoid arthritis (AR) is one of the most commonly found autoimmune arthritis. In clinical practice, this disease has a variety of clinical courses, both in severity, response to treatment, and rate of recurrence. Various components of the immune system have a role in the pathogenesis of rheumatoid arthritis. The prevalence of Th1 celss in the pathogenesis of AR is still controversial. Previous studies demonstrated differences in the predominant Th1 cells in circulation in the course of AR. Increased Th1 activation persisted in patients with advanced AR even after anti-TNF treatment, despite clinical improvement. FOXP3+ expressing Treg cells are distinguishable from other T Cell subsets such as Tr1, Th3, and CD8+CD28+ which also exhibit suppressor functions. Aim: This study aims to prove the correlation between the percentage of Th1, Treg, ratio of Th1/Treg cells with the degree of disease activity and joint damage in RA patients. Methods: This study used a descriptive-analytic method with 28 samples of rheumatoid arthritis patient subjects at the Rheumatology Polyclinic of Dr. Saiful Anwar Hospital Malang from May 2019 until October 2022. Results: The results of Spearman's correlation analysis of the percentage of Th1 cells, Treg cells, and the ratio of Th1/Treg cells with disease activity (DAS28-LED) showed p=0.414 (p>0,05), p=0.218 (p>0,05), and p=0.106 (p>0,05), respectively.The results of Spearman’s correlation analysis of the percentage of Th1 cells, Treg cells with Modified Sharp Score showed p=0,094 (p>0,05), p=0,246 (p>0,05). There is negative correlation between ratio of Th1/Treg cells to Modified Sharp Score with r=-0,470 and p=0,027 (p<0,05). Conclusion: The conclusion of this study is that there is no correlation between Th1, Treg, and Th1/Treg values with DAS28-LED. There is negative correlation between percentage ratio of Th1/Treg to joint damage (Modified Sharp Score).
