Abstract

Beberapa Faktor menentukan tingkat Derajat keparahan dan Mortalitas pasien Covid-19. Golongan darah ABO mempunyai hubungan yang signifikan terhadap tingkat derajat keparahan dan mortalitas pasien Covid-19. Pemeriksaan Golongan darah ABO dapat memberikan gambaran prognosis serta panduan terapi COVID-19. Tujuan penelitian ini adalah untuk mengetahui hubungan antara golongan darah ABO dengan derajat keparahan dan mortalitas pasien COVID-19 yang dirawat di ICU RSUD Dr. Saiful Anwar. Diambil sampel pasien COVID-19 yang dirawat di ICU RSSA mulai dari bulan Maret 2020 sampai dengan September 2021 dengan sumber data dari rekam medis. Data yang dikumpulkan antara lain karakteristik demografis, komorbid pasien, hasil laboratorium Golongan darah, Modalitas terapi oksigenisasi, serta luaran pasien. Data laboratorium yang diambil adalah data pada hari pertama perawatan pasien di ICU. Didapatkan 225 subyek setelah melalui proses inklusi dan eksklusi dengan angka mortalitas sebesar 53,3%. Rerata kadar CRP pada pasien yang meninggal (16,95 95% CI 15,07 -18,84) lebih tinggi daripada pasien hidup (9,59 95% CI 8,04 – 11,15). Ditemukan bahwa kadar CRP > 14,32 mg/dL (AUC 0,721, sensitivitas 57,5%, Spesifisitas 79%, PPV 78%, NPV 58%) pada hari pertama perawatan di ICU merupakan prediktor mortalitas pada pasien COVID-19. Kadar CRP > 9,87 mg/dL (AUC 0,691, sensitivitas 70,9%, Spesifisitas 61,1%, PPV 70,4%, dan NPV 60,5%) pada hari pertama perawatan di ICU merupakan prediktor kebutuhan ventilasi mekanik invasif pada pasien COVID-19. Didapatkan hubungan yang signifikan antara golongan darah ABO dengan derajat keparahan dan mortalitas pasien COVID-19 yang dirawat di ruang ICU COVID-19. Golongan darah ABO dapat digunakan sebagai prediktor luaran pasien COVID-19 yang dirawat di ICU.

English Abstract

Several factors determine the severity and mortality of Covid-19 patients. ABO blood type has a significant relationship with the severity and mortality of Covid-19 patients. ABO blood group examination can provide an overview of the prognosis and guide to COVID-19 therapy. The purpose of this study was to determine the relationship between ABO blood type and the severity and mortality of COVID-19 patients treated in the ICU of RSUD Dr. Saiful Anwar. Samples were taken of COVID-19 patients who were treated in the ICU RSSA from March 2020 to September 2021 with data sources from medical records. The data collected included demographic characteristics, patient comorbidities, laboratory results, blood type, modalities of oxygenation therapy, and patient outcomes. Laboratory data taken is data on the first day of patient care in the ICU. Obtained 225 subjects after going through the inclusion and exclusion process with a mortality rate of 53.3%. The mean CRP levels in patients who died (16.95 95% CI 15.07 -18.84) were higher than those in living patients (9.59 95% CI 8.04 – 11.15). It was found that CRP levels > 14.32 mg/dL (AUC 0.721, sensitivity 57.5%, specificity 79%, PPV 78%, NPV 58%) on the first day of ICU admission were predictors of mortality in COVID-19 patients. CRP levels > 9.87 mg/dL (AUC 0.691, sensitivity 70.9%, specificity 61.1%, PPV 70.4%, and NPV 60.5%) on the first day of ICU treatment were predictors of the need for invasive mechanical ventilation in COVID-19 patients. There was a significant relationship between ABO blood type and the severity and mortality of COVID-19 patients treated in the COVID-19 ICU. ABO blood type can be used as an outcome predictor of COVID-19 patients admitted to the ICU.
