Abstract
Pendahuluan: Prevalensi obesitas meningkat secara global baik pada dewasa, anak-anak maupun remaja. Akumulasi lemak pankreas dan hati sangat terkait dengan obesitas dan jaringan adiposit viseral. Pada obesitas, jaringan adiposit mengalami perubahan bentuk dan jumlah, selain sel adiposit, juga terdapat sel imun pada jaringan adiposit salah satunya yaitu sel makrofag. Metode: Rancangan penelitian true experimental in vivo test, post test only control group menggunakan tikus jantan Sprague Dawley (Rattus norvegicus) yang memenuhi kriteria inklusi dan eksklusi berusia 10-12 minggu sebanyak 16 ekor terbagi dalam 2 kelompok diberikan diet normal dan TK selama 16 minggu. Pemeriksaan akumulasi lemak jaringan dengan pewarnaan Oil red O. Pemeriksaan M1 dan M2 dengan Imunohistokimia. Uji statistik menggunakan statistik deskriptif, nilai p signifikan bila <0.05. Hasil: Dari kelompok diet TK, seluruhnya memenuhi kategori obesitas (p<0,05). Luas area akumulasi lemak hewan coba kelompok diet normal lebih rendah daripada kelompok diet TK (p<0,05). Ekspresi CD11c dan CD206 pada hewan coba kelompok diet normal lebih rendah daripada kelompok diet TK (p<0,05). Rasio M1/M2 pada hewan coba kelompok diet normal lebih rendah daripada kelompok diet TK (p<0,05). Semakin luas area akumulasi lemak, polarisasi makrofag fenotipe M2 (antiinflamasi) menjadi fenotipe M1(pro-inflamasi) (p<0,05). Kesimpulan: Akumulasi lemak pankreas meningkat seiring terjadinya obesitas. Jumlah makrofag meningkat seiring terjadinya obesitas. Pada obesitas terjadi polarisasi makrofag di jaringan pankreas dari fenotipe makrofag M2 (antiinflamasi) ke fenotipe makrofag M1 (pro-inflamasi). Terdapat hubungan positif signifikan antara akumulasi lemak dengan perubahan rasio makrofag M1/M2
English Abstract
Introduction: The prevalence of obesity is increasing globally in adults, children and adolescents. Pancreatic and hepatic fat accumulation is strongly associated with obesity and visceral adipocytes. In obesity, adipocytes change in shape and number, apart from adipocytes, there are also immune cells in adipocytes, one of which is macrophage cells. Methods: The research design is true experimental in vivo test, post test only control group using male Sprague Dawley rats (Rattus norvegicus) that meet the inclusion and exclusion criteria aged 10-12 weeks as many as 16 rats divided into 2 groups given a normal diet and TK for 16 weeks . Examination of accumulation of tissue fat with Oil red O staining. Examination of M1 and M2 through the expression of CD11c and CD206 with Immunohistochemistry. Statistical test using descriptive statistics, p-value is significant if <0.05. Results: From the kindergarten diet group, all of them met the obesity category (p<0.05). The area of fat accumulation in the normal diet group was lower than the TK diet group (p<0.05). The expression of CD11c and CD206 in the normal diet group was lower than the TK diet group (p<0.05). The M1/M2 ratio in the normal diet group was lower than the TK diet group (p<0.05). The wider the area of fat accumulation, the polarization of M2 phenotype (anti-inflammatory) macrophages became M1 (pro-inflammatory) phenotype (p<0.05). Conclusion: Accumulation of pancreatic fat increases with obesity. The number of macrophages increases with obesity. In obesity, there is polarization of adipocyte tissue macrophages from the M2 (anti-inflammatory) macrophage phenotype to the M1 (pro-inflammatory) macrophage phenotype. There is a significant positive relationship between fat accumulation and changes in the ratio of M1/M2 macrophages
