Abstract

Endometritis adalah peradangan pada lapisan rahim akibat adanya infeksi oleh koloni bakteri saluran genetalia bagian bawah yang naik ke rongga rahim. Endometritis paling sering disebabkan oleh bakteri Streptococcus agalactiae yang merupakan bakteri komesal vagina yang berubah menjadi pathogen akibat peningkatan julah koloni dan perubahan pH vagina. Infeksi oleh Streptococcus agalactiae dapat mengaktivasi makrofag sehingga dapat mengaktivasi sitokin pro-inflamasi seperti TNF-α, IL-1β, IL-6, IL-8 dan menekan sitokin anti-inflamasi seperti IL-10. Endometritis akibat infeksi Streptococcus agalactiae dapat menyebabkan infeksi berat pad ibu dan bayi yang dilahirkan. Pengobatan endometritis memerlukan pemberian antibiotik dengan dosis tinggi dan jangka waktu yang lama sehingga dapat menyebabkan efek samping yang berat dan peningkatan resistensi. Pemanfaatan herbal dengan khasiat antibakteri sangat membantu untuk mencegah efek samping pemberian antibiotik dosis tinggi. Tanaman pegagan (Centella asiatica) adalah tanaman dengan senyawa triterpen yang bermanfaat sebagai antibakteri dan imunomodulator. Penelitian ini menggunakan simplisia pegagan (Centella asiatica) dari UPT Herbal Materia Medica Batu Malang yang diproses menjadi crude extract di laboratorium Farmakologi FKUB. Tujuan penelitian ini adalah untuk mengevaluasi pengaruh pemberian ekstrak etanol pegagan terhadap peningkatan kadar IL-10 dan jumlah koloni bakteri uterus. Jenis penelitian ini adalah True eksperimental dengan pendekatan Randomized Post Test OnIy Control Group Design. Jumlah sampel penelitian adalah 24 tikus putih betina (Rattus norvegicus) model endometritis akibat infeksi Streptococcus agalactiae, terbagi dalam 6 kelompok perlakuan yaitu kelompok kontrol negatif, kontrol positif, dan 4 kelompok perlakuan pemberian ekstrak ethanol pegagan dengan dosis P1 100 mg/Kg BB, P2 200 mg/KgBB, P3 400 mg/KgBB, dan P4 800 mg/KgBB yang diberikan selama 7 hari. Kadar IL-10 diukur dengan metode ELISA dan jumlah koloni bakteri dihitung menggunakan colony counter. Hasil penelitian dianalisis menggunakan Uji One Way Anova dengan syarat data berdistribusi normal dan homogen. . Uji One Way Anova kadar IL-10 pada keenam kelompok perlakuan menunjukkan perbedaan yang signifikan dengan p-Value = 0,008 < =0, 05. Demikian juga jumlah koloni bakteri Streptococcus agalactiae antara kelompok perlakuan tikus betina model endometritis menurun secara signifikan yang ditunjukkan dengan nilai ANOVA p-Value = 0,000; koefisien korelasi= α < =0, 05. Ekstrak etanol pegagan dapat meningkatkan kadar IL-10 dan dapat menurunkan jumlah koloni bakteri Streptococcus agalactiae pada tikus model endometritis

English Abstract

Endometritis is inflammation of the lining of the uterus due to infection by bacterial colonies of the lower genital tract that ascend into the uterine cavity. Endometritis is most often caused by Streptococcus agalactiae bacteria, which is a vaginal commensal bacteria that turns into a pathogen due to an increase in the number of colonies and changes in vaginal pH. Infection by Streptococcus agalactiae can activate macrophages so that it can activate pro-inflammatory cytokines such as TNF-, IL-1, IL-6, IL-8 and suppress anti- inflammatory cytokines such as IL-10. Infection with Streptococcus agalactiae can result in severe infections in both the mother and the baby.Treatment of endometritis requires the administration of high doses of antibiotics over long periods of time, so that it can cause severe side effects and increase resistance. The use of herbs with antibacterial properties is very helpful to prevent the side effects of giving high doses of antibiotics. Gotu kola (Centella asiatica) is a plant with triterpene compounds that are useful as antibacterial and immunomodulatory. This study uses gotu kola (Centella asiatica) simplicia from UPT Herbal Materia Medica Batu Malang, which is processed into crude extract in the pharmacology laboratory of FKUB. The purpose of this study was to evaluate the effect of ethanol extract of Gotu Kola on increasing IL-10 levels and the number of uterine bacterial colonies. This type of research is true experimental with a randomized post-test on-line control group design approach. The study included 24 female white rats (Rattus norvegicus) with endometritis caused by Streptococcus agalactiae infection, divided into six treatment groups: a negative control group, a positive control group, and four treatment groups that received gotu kola ethanol extract at doses of P1 100 mg/Kg BW, P2 200 mg/KgBW, P3 400 mg/KgBW, and P4 800 mg/IL-10 levels were measured by the ELISA method, and the number of bacterial colonies was calculated using a colony counter. The study's findings were analyzed using the One-Way Anova test under the assumption that the data was normally distributed and homogeneous. The One-Way Anova test of IL-10 levels in the six treatment groups showed significant differences with p-Value = 0.008 = 0.05. Similarly, the number of Streptococcus agalactiae bacterial colonies decreased significantly between treatment groups of female rats model of endometritis, as indicated by the ANOVA value. The p-value is 0.000, and the correlation coefficient is 0.05. The ethanol extract of Gotu Kola can increase IL-10 levels and can reduce the number of Streptococcus agalactiae bacterial colonies in endometritis rat models
