Abstract

Pendahuluan: Sel dendritik berperan penting dalam replikasi HIV di tahap awal infeksi ataupun sebagai reservoir infeksi laten. Masuknya HIV ke sel dendritik memerlukan reseptor kemokin di permukaan sel, terutama CCR5. Lipooligosakarida (LOS) dianggap memiliki kemampuan untuk mempengaruhi kadar CCR5 melalui desensitisasi dan internalisasi reseptor. Tujuan penelitian ini untuk menganalisis perbedaan kadar CCR5 pada sel dendritik pasien HIV pasca pemaparan berbagai dosis LOS Neisseria gonorrhoeae dan mengetahui dosis optimum LOS Neisseria gonorrhoeae tersebut dalam menekan kadar CCR5. Metode: Penelitian ini adalah penelitian true experimental, in vitro, post test-only and control group design. Subjek penelitian yang digunakan adalah monocyte- derived dendritic cell matur CD14+ CD83+ dari pasien HIV naif, stadium 1. Sel dendritik telah matur dipapar dengan berbagai dosis LOS Neisseria gonorrhoeae dan diinkubasikan dalam suhu 370C dan 5% CO2 selama 24 jam. CCR5 dihitung kadarnya menggunakan enzyme-linked immunosorbent assay (ELISA). Data diolah dengan software SPSS 24 dengan uji beda median kadar CCR5 antar kelompok dan disajikan dengan Confidence interval (CI) 95%. Hasil: Sel dendritik pasien HIV yang dipapar dengan LOS Neisseria gonorrhoeae dosis 2.5%, 5%, dan 10% menunjukkan kadar CCR5 secara signifikan lebih rendah dibandingkan kontrol negatif (32,99± 8,91 vs 320,98 ± 24,75, p= 0.000). Pada dosis 5% dan 10%, CCR5 juga lebih rendah secara signifikan dibandingkan kontrol negatif (162,04 ± 97,88 vs 320,98 ± 24,75, p= 0.027 dan 230,26 ± 15,79 vs 320,98 ± 24,75, p=0.027), dan secara berurutan memiliki median lebih tinggi dibandingkan dosis 2,5%. Kesimpulan: Terdapat perbedaan kadar CCR5 sel dendritik pasien HIV pasca pemaparan berbagai dosis LOS Neisseria gonorrhoeae. Dosis optimum LOS Neisseria gonorrhoeae yang mampu menekan kadar CCR5 adalah 2.5%.

English Abstract

Introduction: Dendritic cells play an important role in HIV replication in the early stages of infection or as reservoirs of latent infection. HIV entry to dendritic cells requires chemokine receptors at the cell's surface, especially CCR5. Lipooligosaccharide (LOS) is thought to have the ability to modulate CCR5 level through desensitization and internalization of receptors. The aim for this research is to analyze differences in CCR5 levels in dendritic cells of HIV patients after exposure by various doses of LOS Neisseria gonorrhoeae and exploring the optimum dose of LOS Neisseria gonorrhoeae in suppressing CCR5 levels. Method: This research is true experimental research, in vitro, post-test-only and control group design. The Subjects were mature monocyte-derived dendritic cells CD14+CD83+ from naïve HIV, stage 1. Mature dendritic cells were exposed to various doses of LOS Neisseria gonorrhoeae and incubated at 37°C and 5% CO2 for 24 hours. CCR5 levels were quantified using enzyme-linked immunosorbent assay (ELISA). The data was processed using SPSS 24 software with a different test for the median of CCR5 levels between groups and presented with 95% confidence interval (CI). Results: Dendritic cells of HIV patients exposed to 2.5% LOS Neisseria gonorrhoeae showed significantly lower levels of CCR5 compared to negative controls (32.99± 8.91 vs 320.98 ± 24.75, p= 0.000). At a dose of 5% and 10% CCR5 levels were also lower significantly compare to negative controls (162.04 ± 97.88 vs 320.98 ± 24.75, p= 0.027 dan 230.26 ± 15.79 vs 320.98 ± 24.75, p=0.027), and show higher CCR5 levels compare to level from dose of 2.5% respectively. Conclusion: There were differences in CCR5 levels of dendritic cells in HIV patients after exposure to various doses of LOS Neisseria gonorrhoeae. The optimum dose of LOS Neisseria gonorrhoeae that can suppress CCR5 levels is 2.5%.
