Abstract
Diabetes melitus tipe 2 merupakan penyakit metabolik yang terjadi karena adanya disfungsi sel beta pankreas dan resistensi insulin sehingga sel-sel tubuh tidak mampu untuk merespon insulin secara penuh. Penyakit DM tipe 2 memberikan beban ekonomi baik di Indonesia maupun di dunia karena pasien memerlukan pengobatan secara rutin. Penelitian ini bertujuan untuk mengetahui perbedaan cost of illness dari biaya medis langsung pasien DM tipe 2 peserta BPJS rawat jalan di beberapa puskesmas Kota Malang tahun 2019 (sebelum pandemi COVID- 19) dan 2020 (ketika pandemi COVID-19). Analisis biaya dilakukan berdasarkan perspektif penyedia layanan kesehatan. Penelitian ini merupakan penelitian retrospektif dengan rancangan observasional analitik. Pengambilan sampel dilakukan dengan menggunakan teknik non-random sampling dengan metode purposive sampling. Sampel yang diperoleh sebanyak 91 pasien DM tipe 2 peserta BPJS rawat jalan yang aktif berobat minimal 3 bulan dan memiliki data catatan rekam medis lengkap di tahun 2019 dan 2020. Analisis biaya medis langsung meliputi biaya administrasi, biaya pemeriksaan laboratorium, biaya obat dan biaya komplikasi. Hasil penelitian ini menunjukkan rata-rata biaya medis langsung tiap pasien DM tipe 2 pada tahun 2019 (sebelum pandemi COVID-19) sebesar Rp243.967 ± 96.335, sedangkan pada tahun 2020 (ketika pandemi COVID-19) sebesar Rp305.115 ± 110.115. Kedua biaya ini berbeda secara signifikan dengan uji t berpasangan (p=0,000). Kesimpulan pada penelitian ini, terdapat peningkatan biaya dan perbedaan yang signifikan pada total biaya medis langsung pasien DM tipe 2 peserta BPJS rawat jalan tahun 2019 (sebelum pandemi COVID-19) dan 2020 (ketika pandemi COVID-19) di beberapa puskesmas Kota Malang.
English Abstract
Type 2 diabetes mellitus is a metabolic disease caused by pancreatic beta-cell dysfunction and insulin resistance. This condition makes cells throughout the body not fully respond to insulin. Type 2 diabetes mellitus disease provides an economic burden in Indonesia and worldwide because patients require regular treatment. This study aims to determine the difference in direct costs used in 2019 (before the COVID-19 pandemic) compared to 2020 (during the COVID-19 pandemic) for treating type 2 diabetes mellitus in BPJS (the Indonesian National Insurance System) outpatients. The cost analysis was carried out from the healthcare provider's perspective. This study was a retrospective study with an analytical observational design. This study used a non-random sampling technique with a purposive sampling method. We recruited 91 types 2 diabetes mellitus outpatients participating in BPJS who were in active treatment for at least 3 months and had complete medical record data in 2019 and 2020. The types of costs included in this study were registration charges, laboratory fees, drug costs, and complication treatment costs. The results showed that the average total direct medical cost of type 2 diabetes mellitus outpatients participating in BPJS at several Malang city primary healthcare centers in 2019 (before the COVID-19 pandemic) was IDR 243,967 ± 96,335, and in 2020 (during the COVID-19 pandemic) it was IDR 305,115 ± 110,115. These two costs differ significantly with the paired sample t- test (p=0.000). It can be concluded that there is an increase in costs and a significant difference in the total direct medical cost of type 2 diabetes mellitus outpatients participating in BPJS at several Malang city primary healthcare centers in 2019 (before the COVID-19 pandemic) and 2020 (during the COVID-19 pandemic)
