Abstract

Latar Belakang: Stres oksidatif berperan penting dalam patogenesis fibrosis hati. Malondialdehid (MDA) merupakan produk akhir dari peroksidasi lipid pada membran sel, yang paling sering digunakan sebagai penanda stres oksidatif biologis. Glutation Peroksidase (GPx) adalah enzim antioksidan endogen yang berperan menjaga integritas jaringan dari kerusakan oksidatif. Kurkumin sebagai antioksidan,antiinflamasi, antifibrotik dipercaya sebagai salah satu terapi baru yang dapat memicu regresi fibrosis hati. Saat ini belum banyak penelitian mengenai regresi fibrosis hati yang digambarkan dengan perbaikan kadar MDA jaringan hati dan aktivitas GPx jaringan hati terhadap pemberian kurkumin. Tujuan: Mengetahui hubungan antara pemberian kurkumin terhadap kadar MDA dan aktivitas GPx jaringan hati pada tikus fibrosis hati. Metode: Penelitian eksperimental in vivo pada tikus jantan Rattus novergicus strain wistar dengan studi acak lengkap. Terdapat 8 kelompok, masing-masing terdiri dari 4 tikus. Kelompok Kontrol Positif (K-pos) dipapar CCl4 (karbon tetraklorida) 1cc/kgbb 2x perminggu intraperitoneal selama 9 minggu untuk membentuk fibrosis F3. Kelompok Kontrol Negatif (K-neg) diinjeksi NaCl 0,9%. Setelah diinjeksi CCl4, Kelompok Kontrol (KK) diberikan plasebo dan Kelompok Perlakuan (KP2, KP5 dan KP9) diberikan kurkumin 200mg/kgbb per oral masing-masing selama 2, 5 dan 9 minggu. Dilakukan analisis kadar MDA dan aktivitas GPx jaringan hati dengan menggunakan uji Mann-Whitney, uji t independen, uji pengaruh dan lama pemberian kurkumin dengan uji Kruskal-Wallis dan ANOVA. Hasil: Terdapat peningkatan kadar MDA jaringan hati namun tidak signifikan secara statistik dan peningkatan signifikan aktivitas GPx jaringan hati pada tikus fibrosis hati. Pemberian kurkumin menurunkan kadar MDA jaringan hati yang signifikan pada KP5 dan KP9 dan meningkatkan aktivitas GPx jaringan hati yang signifikan pada KP5. Kesimpulan: Terdapat peningkatan kadar MDA dan peningkatan aktivitas GPx jaringan hati pada tikus fibrosis hati. Terdapat penurunan kadar MDA dan penurunan aktivitas GPx jaringan hati pada regresi fibrosis hati spontan. Pemberian kurkumin menurunkan kadar MDA jaringan hati secara signifikan setelah minggu ke-5 dan meningkatkan aktivitas GPx jaringan hati secara signifikan hanya pada minggu ke-5. Terdapat korelasi antara lama pemberian kurkumin dengan penurunan kadar MDA dan penurunan aktivitas GPx jaringan hati pada regresi fibrosis hati.

English Abstract

Background: Oxidative stress plays an important role in the pathogenesis of liver fibrosis. Malondialdehyde (MDA) is an end product of lipid peroxidation in cell membrane, the most common biologic marker for oxidative stress. Glutathione Peroxidase (GPx) is an endogenous antioxidant enzyme which maintaining tissue integrity from oxidative damage. Curcumin as an antioxidant, anti-inflammatory, antifibrotic is believed to be one of the new therapy that can lead liver fibrosis regression. At present there have not been many studies about liver fibrosis regression which is represented by improvement in MDA level and GPx activity of liver tissue to the administration of curcumin. Objective: To evaluate the relationship between administration of curcumin to MDA levels and GPx activity of liver tissue in rat model of liver fibrosis. Methods: This is an experimental in vivo study in male rats Rattus novergicus wistar strain with complete randomized study. There are 8 groups, each group consisting of 4 rats. K-pos group was exposed to CCl4 1 cc/kg intraperitoneally 2 times per week for 9 weeks to form F3 fibrosis. K-neg group was injected Normal Saline. After injection of CCl4, the Control Group (KK2, KK5, KK9) was given a placebo for 2, 5, 9 weeks respectively and the Treatment Group (KP2, KP5, KP9) was given curcumin orally 200 mg/kgbw for 2, 5 and 9 weeks, respectively. Analysis of MDA levels and GPx activity of liver tissue was conducted by Mann-Whitney test, independent t-test, effect of curcumin and the duration of curcumin administration by Kruskal-Wallis test and ANOVA. Result: MDA levels of liver tissue was increased but not statistically significant and GPx activity of liver tissue was increased significantly in rat model of liver fibrosis. Curcumin administration significantly reduced MDA levels in KP5 and KP9 and significantly increased GPx activity of liver tissue in KP5. Conclusion: There were an increased of MDA levels and GPx activity of liver tissue in rat model of liver fibrosis. There were a decreased of MDA levels and GPx activity of liver tissue in spontaneous regression of liver fibrosis. Curcumin administration significantly decreased liver tissue MDA levels after 5 weeks and significantly increased liver tissue GPx activity only at week 5. There was a correlation between the duration of curcumin administration and decreased of MDA levels and GPx activity of liver tissue in regression of liver fibrosis.
