Abstract

Pendahuluan: Inflamasi adenoid ditandai dengan ditemukannya pembesaran ukuran adenoid atau hipertrofi adenoid. Penelitian yang mendukung bahwa IL-6 merupakan dasar patofisiologi terjadinya hipertrofi adenoid telah banyak dilakukan sehingga dibutuhkan terapi yang dapat mengurangi peran IL-6. Saat ini, kortikosteroid intranasal menjadi pilihan terapi pada hipertrofi adenoid karena memiliki efek anti inflamasi. Terdapat penelitian mengenai manfaat penggunaan kortikosteroid intranasal terhadap penderita hipertrofi adenoid, bahwa terdapat penurunan kadar IL-6 pada jaringan adenoid hipertrofi setelah pemberian kortikosteroid intranasal, tetapi pengaruhnya terhadap kadar IL-6 serum tidak diteliti. Tujuan: Mengetahui pengaruh pemberian mometasone furoate semprot hidung terhadap kadar IL-6 serum pada penderita hipertrofi adenoid. Metode: Penelitian the one group pre and post test design ini melibatkan 16 subyek. Subyek akan menjalani pemeriksaan foto polos skull lateral soft tissue juga dilakukan pengambilan darah tepi untuk menghitung kadar IL-6 serum. Kemudian subyek akan diberikan terapi mometasone furoate semprot hidung selama 6 minggu. Subyek akan dilakukan pengambilan darah tepi untuk menghitung kadar IL-6 serum setelah 2 minggu dan 6 minggu serta evaluasi foto polos skull lateral soft tissue setelah 6 minggu. Hasil: Semua subjek penelitian mengalami penurunan kadar IL-6 serum dan peningkatan nilai pada foto polos skull lateral soft tissue setelah mendapatkan perlakuan selama 6 minggu. Terdapat perbedaan bermakna antara kadar IL-6 serum sebelum dan sesudah perlakuan baik pada minggu kedua maupun minggu keenam (p=0.00). Terdapat perbedaan bermakna antara nilai foto polos skull lateral soft tissue sebelum dan sesudah perlakuan (p=0.00). Berdasarkan uji Pearson, nilai kadar IL-6 serum darah berkorelasi secara signifikan yang bersifat negatif (p<0,05) dengan foto polos skull lateral sebelum dan sesudah perlakuan dengan hasil -0,819 dan -0,692. Kesimpulan: Mometasone furoate semprot hidung dapat menurunkan kadar IL-6 serum pada penderita hipertrofi adenoid secara bermakna yang diikuti dengan mengecilnya ukuran adenoid serta penurunan gejala dan keluhan yang dialami oleh penderita hipertrofi adenoid

English Abstract

Introduction: Adenoid inflammation is characterized by enlargement of adenoid or adenoid hypertrophy. Research stating that IL-6 is the pathophysiological basis for the occurrence of adenoid hypertrophy has been done, so that therapy is needed to reduce the role of IL-6. Intranasal corticosteroids are the treatment of choice for adenoid hypertrophy since they have the ability as an anti-inflammatory. There is research on the benefits of intranasal corticosteroid for patients with adenoid hypertrophy, stated that there was a decrease in IL-6 levels in adenoid tissue after administration of intranasal corticosteroids, but its effect on IL-6 serum levels was not studied. Purpose: To understand the Effect of Mometasone Furoate on Interleukin 6 Serum Levels in Patient with Adenoid Hypertrophy. Methods: This is the one group pre and post test design study which is involving 16 subjects. Subjects will undergo soft tissue skull lateral plain x- ray. There is also procedure to count IL-6 serum levels from peripheral blood. Then the subjects will be treated with mometasone furoate nasal spray for 6 weeks. Subjects will be reassessed IL-6 serum levels from peripheral blood after 2 weeks and 6 weeks, and evaluation of soft tissue skull lateral plain x-ray after 6 weeks. Results: The IL-6 serum levels decreased in all subjects and enhancement of value in soft tissue skull lateral plain x-ray after treatment for 6 weeks. There were significant differences between IL-6 serum levels before and after treatment both in the second week and sixth week (p = 0.00). There were significant differences between the value of soft tissue skull lateral plain x-ray before and after treatment (p = 0.00). Based on the Pearson test, IL-6 serum levels correlated significantly negatively (p <0.05) with soft tissue skull lateral plain x-ray before and after treatment with results is -0.819 and -0.692. Conclusion: Mometasone furoate nasal spray can significantly reduce IL-6 serum levels in patients with adenoid hypertrophy, accompanied by reduction of adenoid size and decreasing symptoms in patients with adenoid hypertrophy.
