Abstract

Latar belakang: Karies gigi merupakan penyakit di rongga mulut yang prevalensinya masih cukup tinggi di Indonesia. Pembuangan jaringan karies aktif dan pembuatan restorasi dilakukan sebagai upaya mengurangi pertumbuhan bakteri kariogenik Streptococcus mutans (S. mutans). Semen zinc oxide eugenol merupakan salah satu bahan tambahan pada restorasi gigi sebagai liner maupun basis. Namun, eugenol dalam bahan tersebut menunjukkan toksisitas terhadap sel fibroblas pulpa bahkan dalam konsentrasi terendah. Propolis mengandung flavonoid, asam fenol, caffeic acid phenethyl ester (CAPE) memiliki kemampuan antibakteri dan anti-inflamasi sehingga dapat merangsang dentin reparatif. Sedangkan zinc oxide berdasarkan ukuran partikel, morfologi, konsentrasi, aglomerasi, mempengaruhi penghambatan pertumbuhan bakteri. Tujuan: Mengetahui perbedaan daya antibakteri antara nano zinc oxide propolis dan zinc oxide eugenol terhadap pertumbuhan S. mutans dengan metode difusi sumuran. Metode: Propolis yang digunakan dalam penelitian adalah propolis komersial “Melia” (BPPOM RI POM. TI 124 646 701) dan nano zinc oxide disintesis melalui proses mechanical milling selama 50 jam. Pengujian antibakteri dibagi menjadi 5 kelompok perlakuan, yaitu nano zinc oxide dengan variasi konsentrasi propolis (3,125%, 6,25%, 12,5%), zinc oxide eugenol (kontrol positif), aquades (kontrol negatif) dilakukan pengamatan diameter zona hambat setelah inkubasi 24 jam dan 48 jam. Hasil: Nano zinc oxide propolis konsentrasi 3,125%, 6,25%, 12,5%, dan zinc oxide eugenol setelah inkubasi 24 jam menghasilkan rerata diameter zona hambat masing-masing yaitu 11,95 mm, 14,86 mm, 17,20 mm, dan 19,36 mm, setelah inkubasi 48 jam terjadi penurunan rerata diameter zona hambat masing-masing menjadi 9,95 mm, 12,82 mm, 15,14 mm, dan 17,44 mm. Uji Kruskal Wallis menunjukkan adanya perbedaan bermakna (p=0,000) antar kelompok perlakuan. Dilanjut uji Mann-Whitney menunjukkan adanya perbedaan bermakna (p<0,05) antara daya antibakteri nano zinc oxide propolis dengan zinc oxide eugenol, kecuali pada kelompok nano zinc oxide propolis konsentrasi 12,5% tidak terdapat perbedaan bermakna dengan zinc oxide eugenol setelah inkubasi 24 jam (p>0,05). Kesimpulan: Rerata diameter zona hambat kelompok nano zinc oxide propolis konsentrasi 3,125%, 6,25%, 12,5% setelah inkubasi 24 jam dan 48 jam tidak lebih besar dari kelompok zinc oxide eugenol.

English Abstract

Background: Dental caries is an oral disease whose the prevalence is still quite high in Indonesia. The active caries tissue removal and the making of dental restoration were an effort to reduce the growth of the cariogenic bacteria Streptococcus mutans (S. mutans). Zinc oxide eugenol cement is one among the additional material in dental restorations as a liner or base. However, eugenol in the material shows toxicity to pulp fibroblasts cells even in the lowest concentration. Propolis contains flavonoids, phenolic acids, caffeic acid phenethyl ester (CAPE) which has antibacterial and anti-inflammatory properties so that it can stimulate reparative dentin. While zinc oxide based on particle size, morphology, concentration, agglomeration, affects the inhibition of bacterial growth. Objective: To determine the difference of antibacterial power between nano zinc oxide propolis and zinc oxide eugenol against the growth of S. mutans by the well diffusion method. Method: Propolis that used in this research is commercial propolis “Melia” (BPPOM RI POM. TI 124 646 701) and nano zinc oxide was synthesized through a mechanical milling process for 50 hours. Antibacterial testing was divided into 5 treatment groups, namely nano zinc oxide with various concentrations of propolis (3,125%, 6,25%, 12,5%), zinc oxide eugenol (positive control), distilled water (negative control) and observed the inhibition zone diameter after 24 hours and 48 hours incubation. Result: Nano zinc oxide propolis in 3,125%, 6,25%, 12,5% concentration, and zinc oxide eugenol after 24 hours incubation resulted in the mean inhibition zone diameter respectively 11,95 mm, 14,86 mm, 17,20 mm, and 19,36 mm, after 48 hours of incubation decreased the mean inhibition zone diameter to 9,95 mm, 12,82 mm, 15,14 mm, and 17,44 mm. The Kruskal Wallis test showed a significant difference (p=0.000) between treatment groups. Followed by Mann-Whitney test showed significant differences (p<0,05) between the antibacterial power of nano zinc oxide propolis and zinc oxide eugenol, except the group of nano zinc oxide propolis with 12,5% concentration was not significantly different with zinc oxide eugenol after incubation 24 hours (p>0,05). Conclusion: The mean inhibition zone diameter of the nano zinc oxide propolis group with a concentration of 3,125%, 6,25%, 12,5% after 24 hours and 48 hours incubation was not greater than the zinc oxide eugenol group.
