Kesintasan Dua Tahun Pasien Gagal Ginjal Tahap Akhir dengan Komorbid Diabetes Melitus yang Menjalani Continuous Ambulatory Peritoneal Dialysis

Abstract

Angka kejadian penyakit ginjal kronik masih terus meningkat dari tahun ke tahun dan hingga saat ini masih menjadi masalah di masyarakat global. Penyakit ginjal kronik juga menempati peringkat 18 sebagai penyebab kematian terbanyak pada tahun 2010. Diabetes melitus menjadi penyebab terbanyak kedua serta menjadi komorbid terbanyak kedua yang dialami oleh pasien gagal ginjal tahap akhir. Continuous Ambulatory Peritoneal Dialysis (CAPD) menjadi salah satu pilihan terapi pengganti ginjal yang ekonomis. Penelitian ini bertujuan untuk mengetahui pengaruh komorbid diabetes melitus terhadap kesintasan dua tahun pasien gagal ginjal tahap akhir. Penelitian ini merupakan penelitian retrospective cohort dengan menggunakan data rekam medis pasien CAPD periode Agustus 2019 - Juli 2021 di RSUD Dr. Saiful Anwar Malang. Studi ini melibatkan 123 pasien gagal ginjal tahap akhir yang menjalani CAPD di RSUD Dr. Saiful Anwar Malang. Analisis penelitian ini menggunakan uji Multiple Logistic Regression serta analisis survival dengan menggunakan Kurva Kaplan-Meier. Hasil dari analisis didapatkan hubungan bermakna antara komorbid diabetes melitus dengan kesintasan dua tahun serta risiko mortalitas pasien. Hal tersebut disimpulkan dari p-value= 0.001 (p < 0.05) pada uji Multiple Logistic Regression serta model kurva Kaplan-Meier. Odd ratio pasien gagal ginjal tahap akhir dengan komorbid diabetes melitus yang menjalani CAPD yaitu 5.44 kali. Kesimpulannya, komorbid diabetes melitus menurunkan survival rate atau meningkatkan risiko mortalistas pada pasien gagal ginjal tahap akhir yang menjalani CAPD.

English Abstract

The incidence of chronic kidney disease continues to increase from year to year and is still a problem in the global community. Chronic kidney disease also ranks 18th as the most common cause of death in 2010. Diabetes mellitus is the second most common cause and the second most common comorbidity experienced by patients with end-stage renal disease (ESRD). Continuous Ambulatory Peritoneal Dialysis (CAPD) is an economical choice of renal replacement therapy. This study aimed to determine the effect of comorbid diabetes mellitus on two-year survival in end-stage renal disease patients. This study was a retrospective cohort study using medical record data of CAPD patients for the period August 2019 - July 2021 in Dr. Saiful Anwar RPH Malang. This study involved 123 end-stage renal disease patients undergoing CAPD in Dr. Saiful Anwar RPH Malang. The analysis of this study used Multiple Logistic Regression test and for the survival analysis, this study used Kaplan-Meier curve. The results of the analysis showed a significant relationship between comorbid diabetes mellitus with two-year survival and the risk of patient mortality. It was concluded from p-value = 0.001 (p < 0.05) in Multiple Logistic Regression test and the Kaplan-Meier curve model. The odds ratio of patients with end-stage renal disease with comorbid diabetes mellitus undergoing CAPD is 5.44 times. The conclusion is comorbid diabetes mellitus reduces the survival rate or increases the risk of mortality in end-stage renal disease patients undergoing CAPD.
